重度眼表热烧伤的综合治疗 被引量:1

摘要 目的重点研究总结眼表热烧伤急性期、营养紊乱期的综合治疗。方法按Roper_Hall分度法把眼表烧伤分为4度,并根据烧伤的病理过程分为3期:急性期、营养紊乱期、瘢痕期。急性期全身大量应用皮质类固醇、维生素C及局部肝素治疗。营养紊乱期局部应用自身血清,瘢痕期促其愈合等综合治疗措施。结果Ⅲ度以上烧伤24眼中保留眼功能者23眼,1眼形成角膜葡萄肿者应患者要求行眼球摘除术。结论眼表热烧伤处理不当必然导致感染、融解坏死,甚至穿孔、或发生眼内炎等严重并发症。有效的综合治疗是保存眼球及为瘢痕期后治疗创造有利条件的保证。 Objective Emphasis on studying and making a conclusion on the comprehensive treatment for thermal-burned eye at acute period, nutrition irregular period. Methods With Roper- Hall way, separated thermal-burned eyes into four degrees. And according to burn injury pathological process, separated it into three periods: acute period, nutrition irregular period and scar period. Acute period: more glucocorticosteroids, Vitamin C should be applied to the whole body and apply Heparin to partial body. Nutrition irregular period: patient' s own blood serum should be applied to the partial body. Scar period:comprehensive treament should be applied to make it recover. Results Of 24 thermalburned eye,23 eyes had functions, cornea staphyloma was formed on one eye. Conclusions Eye thermal-burn is a complicated and long pathological process. If the treatment is not correct, that will induce infection on the wound and dissolve, even a perforation in the eyeball. Therefore, making a serious scientific and effective comprehensive treatment system is guarantee for saving eyeball and creating effective condition for the treatment of scar period.
