摘要
目的 评价地塞米松缓释系统在可调缝线小梁切除术对难治性青光眼的效果。方法 观察组难治性青光眼34例43眼，采用可调缝线小梁切除术，术中应用地塞米松缓释系统，术后根据滤泡情况及前房深度适时调整可调缝线的松紧度。对照组为单纯的原发性开角型或闭角型青光眼32例41眼，行单纯小梁切除术。术后随访1～18月，分析比较两组术后眼压、滤过泡及前房深度。结果 术后第1天，观察组浅前房发生率为9．30％，明显低于对照组的26．8％（P〈0．05）。术后第7天，观察组浅前房发生率4.65％，明显低于对照组的18．83％（P〈0．05）。术后12月随访到30眼（观察组17眼，对照组13眼），观察组功能滤过泡占94．12％，多于对照组的53．85％（P〈0．005）。结论 可调缝线小梁切除术联合地塞米松缓释系统与传统小梁切除术相比，手术成功率明显提高、并发症少，是治疗难治性青光眼较为理想的方法。
Objective To evaluate the efficacy and feasibility of dexamethasone delivery system for surgery of refractory glaucoma, Methods 43 eyes underwent trabeculectomy combined with dexamethasone delivery system and adjustable suture （observed group） ; and 41 eyes underwent trabeculectomy as control to compare the postoperative intraocular pressure（10P）, filtration pad and the depth of anterior chamber between the two groups. Results The incidence rates of shallow anterior chamber on the first day and the seventh day after operation in observed group and control group were 9.3% ,4.65% and 26.8%, 18.83% respectively.30 eyes were followed up 1 year later, and the success rates of filtration pad were 94.12% and 53.85% （ P 〈 0.05）. The definition of success was the 10P being less than 21 mnlHg postoperatively without using antiglaucoma drugs. The success rate was 94.1% and 69.3% respectively in two groups 1 year after operation. The postoperative intraocular pressure on 6 months and 12 months of observed group and control group were 15. 08 ± 4.17mmHg, 15.87 ± 4.08mmHg compared with 18.79 ± 5.56mmHg, 18.96 ± 4.93 mmHg （t=3.60,P〈0.01;t =2.26;P〈0.05）.Conclusion The rational applications of dexametbasone delivery system and adjustable suture in trabeculectomy on refractory glaucoma may improve the surgical success rate and reduce the complications.
