早期干预促进中重度新生儿缺氧缺血性脑病智能发育的远期疗效观察 被引量:5

摘要 【目的】观察早期智能干预促进中重度新生儿缺氧缺血性脑病(hypoxic—iscbemic encephalopathy,HIE)患儿智能发育、防治神经系统后遗症的远期效果。【方法】将同期住院的52例中重度HIE患儿随机分为早期干预组27例、对照组25例。两组接受相同的HIE常规治疗,干预组从生后7~10d开始接受早期智能干预。两组患儿均按双盲原则.于2岁时行Bayley婴幼儿发育量表(BSID)、5岁时行韦氏学龄前儿童智力量表(WPPSI)测试,以评价早期智能干预促进中重度HIE患儿神经康复的远期疗效。【结果】影响婴幼儿智能发育的有关生物学及环境因素分析,干预组与对照组比较差异无显著性(P〉0.05)。2岁时Bayley智测。干预组心理发展指数和运动发育指数显著高于对照组(P均〈0.05)。5岁时WPPSI智测.干预组总智商、言语智商和操作智商均显著高于对照组(P均〈0.05)。【结论】早期智能干预可促进中重度HIE患儿智能发育,促进神经康复.明显改善患儿的远期预后。 [Objectives] To study the long-time effect of early intellectual intervention on intellectual development of infants with moderate and severe hypoxie-ischemic encephalopathy(HIE). [Methods] 52 infants with moderate and severe HIE were randomly divided into early intervention group (n=27) and control group (n=25). The infants of the intervention group received early intellectual intervention begining at the age oT 7- 10 days. All infants in two groups received the same routine treatment during neonatal period. They received Bayley scale of infant development (BSID) test at the age of 2 years, Wechsler preschool and primary scale of intelligence (WPPSI)test at the age of 5 years. The intelligence tests were double blind. [Results] There was no significant difference in biological and environmental factors between inter- vention and control groups. At the age of 2 years, the average score of mental development indexs (MDI) and psychomotor mental development indexs(PDI)of BSID in intervention group were significantly higher than those of the control group (t =2. 34,2.06; P〈0.05). At the age of 5 years the average score of full-scale intelligence quotient(FIQ) , verbal intelligence quotient (VIQ) and performance intelligence quotient(PIQ) of WPPSI in intervention group were significantly higher than those of the control group(t=2.03-2.23;P〈0.05). [Conclusion] Early intellectual intervention can improve intellectual development of infants with moderate and severe HIE,and can significantly improve their long-term prognosis.
