摘要
目的 探讨颅脑损伤患者性激素动态变化与伤情、预后的相关性。方法 应用性激素定量测定的方法.动态检测100例不同伤情的颅脑损伤患者血浆中性激素的含量,分析患者性激素含量与格拉斯哥昏迷分级(GCS)、格拉斯哥预后分级(GOS)的关系。结果 各组伤后12h内血清雌二醇、孕酮和睾酮升高,而GCS≤12分(中、重型)组雌二醇伤后1d和3d内持续增高,GCS为3~8分(重型)组在受伤后3d睾酮降低,与其他各组比较差异有显著意义(P〈0.05);忤激素含量与GOS呈正相关。结论 颅腑损伤患者性激素的定量测定有助于早期、准确地判断伤情与预后;早期雌二醇增高对脑组织有一定的保护作用;性激索减少是重型颅脑损伤患者死亡的重要因素。
Objective To study the correlation between the dynamic changes of sex hormones and the severity and prognosis of head injury,and to investigate the role of sex hormones in traumatic brain injury. Methods The dynamic changes of sex hormones were quantitatively dynamic measured in 100 patients with head injuries, while the relationships between glasgow coma score ( GCS), glasgow outcome score (GOS) and the concentration of sex hormones were also studied. Results The estradioi, progesterone and testosterone levels in serum increased within 12 hours after trauma in all groups. Estradiol increased markedly contimnously within 1,3 (lays in GCS≤12 group, testosterone levels in serum decreased within 3 days after trauma in GCS = 3- 8 group, with a significant differnee compared with others groups. The concentration of sex hormones correlated positively with GOS. Conehtsions The measurement of sex hormones may be useful to judge the severity and prognosis of head injury in earlystage. The increases of estradiol in the early stage after traumatic brain injury may exert powerful protective effects on brain tissties, Decreases of sex hormones are importam factors leading to death of patients of traumatic brain injury.
