远程细胞学诊断的准确性分析

摘要 目的:建立全国范围的远程细胞学诊断网络,初步探讨远程细胞学诊断的准确性。方法:在全国8个城市13所医院或医学院建立远程细胞学诊断网络,60份细胞学标本的数码图像资料通过专门的网页或电子邮件发送给参加单位。参加者根据屏幕图像作出远程诊断。结果:共收到456份远程诊断报告,与传统光学显微镜检查相比,符合率达86%~97%,平均94%。远程诊断自信度达96%,图像满意度达98%。结论:利用互联网传送细胞学图像,其诊断的准确度与光学显微镜相似,这种模式还可应用到全国范围的远程教学和形态学诊断的质量控制。 Objectives To set up a tele-cytology network in China and evaluate the diagnostic accuracy of the tele-cytology. Methods A tele-cytology project was initiated among 13 hospitals in 8 cities of China. The digital images of 60 cytology samples along with the case information were displayed on web page or transmitted as E-mail attachment. Participants viewed the representative microscopic images and rendered a tele-diagnosis. Results A total of 456 responses were obtained. When the tele-diagnosis was compared to the traditional light microscopic diagnosis, the diagnostic agreement ranged from 86% to 97% with the average value of 94%. The diagnostic certainty was 96% and the accept- ability of the image quality reached 98%. Conclusions The accuracy of diagnosis by tele-cytology is similar to that by conventional light microscopy. The transmission of the cytology digital images via the Internet is the first step in establishing a tele-cytology system, which may be expanded into the web-based education program and may act as a professional quality assurance in China as well.
