摘要
目的:观察光动力疗法作用于肌组织时的损伤演变特点,观察光及光敏剂剂量改变对损伤深度的影响。方法:实验于2004-06/2005-03在解放军总医院激光科实验室进行。①取新西兰白兔12只,采用632.8nm激光,功率密度200mW/cm2PSD-007光敏剂,剂量10mg/kg,在光敏剂静脉给药后5min开始对兔下肢肌肉进行照光,时间15min。于照光后即刻、3d、3周、6周后取材进行大体和光学显微镜下观察。②取新西兰白兔20只,分为4组,每组5只。高功率高剂量组:功率密度400mW/cm2,PSD-00710mg/kg;高功率低剂量组:功率密度400mW/cm2,PSD-007剂量10mg/kg;低功率低剂量组:功率密度200mW/cm2,PSD-007剂量10mg/kg;低功率高剂量组:功率密度400mW/cm2,PSD-007剂量10mg/kg。激光辐照兔下肢肌肉,时间15min。于照光后3周行大体及镜下观察病理组织特点,并在镜下用显微测微尺测量损伤深度。结果:32只兔均进入结果分析。①兔照光区域大体观察及组织病理学特点:照光后即刻照光区域内未见明显改变。照光后3d,损伤区域内相继出现肌细胞变性、坏死,最终被吸收的改变,同时伴有炎性细胞浸润和胶原纤维增生。②兔组织损伤深度测量结果:功率密度增加,损伤深度增加,高功率高剂量组组织损伤深度最重为(3.87±0.40)mm,低功率低剂量组组织损伤深度最轻为(2.36±0.33),而光敏剂剂量改变对损伤深度没有明显改变。结论:光动力疗法对家兔肌肉组织引起的病理改变为损伤到修复的过程;损伤深度主要由激光剂量决定。
AIM: To observe the characteristicS of the change of the muscular injury caused by photodynamic therapy (PDT), and observe the effect of light and photosensitizer dose changes on the damage depth.
METHODS: The experiment was carried out in the laboratory of Department of Laser Medicine, General Hospital of Chinese PLA between June 2004 and March 2005. (1) Twelve New Zealand rabbits' muscle in the lower extremity was irradiated by 632.8 nm laser with a power density of 200 mW/cm^2 for 15 minutes, which was performed at 5 minutes after photosensitizer PSD-007 (10 mg/kg) was intravenously injected. Macroscopic and light microscopic observations were performed immediately, at 3 days, 3 and 6 weeks after irradiation. (2) Twenty New Zealand rabbits were divided into 4 groups with 5 rabbits in each group: high power and high dosage group (power density: 400 mW/cm^2, PSD-007:10 mg/kg), high power and low dosage group (power density: 400 mW/cm^2, PSD-007: 5 mg/kg), high power and high dosage group (power density: 200 mW/cm^2, PSD-007:5 mg/kg), high power and high dosage group (power density: 200 mW/cm^2, PSD-007: 10mg/kg). In all the four groups, the muscle was irradiated for 15 minutes. Histological characters were observed at 3 weeks after irradiation and the damage depth was measured with micrometer.
RESULTS: Totally 32 rabbits were involved in the analysis of results. (1) Results of gross observation and pathological characters in the irradiated area: There was no obvious change immediately after irradiation. The irradiated muscular cell became change of degeneration, necrosis and being absorbed subsequently with the inflammatory cell infiltration and collagen proliferation at 3 days after irradiation. (2) Measured results of injured depth: The rise of light dose increased the damage depth. The injured depth was the severest in the high power and high dosage group [(3.87 ±0.40)mm], but the lightest in the low power and low dosage group [(2.36 ±0.33) mm]. The dosage changes had no obviously influence on the injured depth.
CONCLUSION: The pathological change in muscle tissue caused by PSD-007-PDT is a reparative process after damage, the depth of which is mainly determined by light dose.
