摘要
探索了一种旨在降低胸腰段椎弓根螺钉植入失败率的定位手段。用钢丝环扎标本的椎弓根,定位针入骨质处夹止血钳拍前后位X线片。定位针被标记止血钳分为骨内段和骨外段,通过判断骨内段中点与钢丝环的关系即可了解定位针在椎弓根内的位置,此点与钢丝环的最小距离大于4.5mm可保证螺钉在理想的位置。临床应用时以椎弓根内侧壁和此点距离大于3mm为标准。实验和临床应用均证明此方法的高度准确性。采用本研究所建议的定位手段结合空心螺钉可有效地提高螺钉准确植入率。
The purpose of this study is to set up a new practicaltechnique to reduce the failure of transpedicular inseration. The narrow portion of the pedicle were marked with stainless wires and guide wires through pedicle to vertebrae were marked with forceps at the entry point. On AP view of X-ray film,the guide wire were divided into intravertebrae and extravertebrae part, the relationship between mark wire circle and middle point of intravertebrae of guide wire indicates the position of guide wire through the pedicle on the cornal plane. If the least distance between this point and the wire was more than 4. 5-mm,the screw will be correctly inserted , while 3mm baseline between this point and the inner cortical wall of pedicle was used in the operation. High accuracy were confirmed both in experiments and operations. An ideal position of the transpedicular screw can be obtained by means of the technique proposed combining the central cannlated screw system.Author's address(Department of Orthopaedic Surgery ,Changzheng Hospital , Shanghai ,200003)
