PTEN mRNA、SMAD4 mRNA在肝外胆管癌中的表达及临床意义 被引量:1

Expression and clinical significance of PTEN mRNA and SMAD4 mRNA in extrahepatic cholangiocarcinoma

摘要 目的通过检测第10号染色体磷酸酶和张力蛋白同源丢失基因(PTEN)mRNA和TGF的细胞内信号转导蛋白家族(SMAD)mRNA在肝外胆管癌中的表达,研究它们在肝外胆管癌发生发展过程中的表达特点、相互关系以及对早期诊断及预后判断的临床价值。方法应用原位杂交染色法检测42例肝外胆管癌中PTEN mRNA、SMAD4 mRNA的表达,并以同期20例慢性胆管炎作对照。结果(1)PTEN mRNA、SMAD4 mRNA基因阳性表达率分别为57.14%(24/42)、61.90%(26/42),均显著低于对照组(均为100%)。(2)经统计学分析,PTEN mRNA、SMAD4mRNA的表达都分别与肝外胆管癌的临床病理分期及预后显著相关(P<0.05);与肝外胆管癌的组织学分级显著相关(P<0.05);SMAD4 mRNA的表达还与肝外胆管癌是否发生浸润转移显著相关(P<0.05)。(3)PTEN mRNA与SMAD4 mRNA在肝外胆管癌中的表达呈正相关。结论PTEN、SMAD4基因在肝外胆管癌的发生发展过程中可能起重要作用。联合检测PTEN mRNA、SMAD4 mRNA在肝外胆管癌中表达有助于反映肝外胆管癌生物学特性、为预后判断提供参考指标,并为探索肝外胆管癌早期诊断和干预性基因治疗提供相关的实验依据。 Objective To investigate the correlation between the expressions of PTEN mRNA and SMAD4 mRNA in extrahepatic bile duct cancer and the development of the carcinoma. Methods In situ hybridization(ISH) was used to examine the expressions of PTEN mRNA and SMAD4 mRNA in 42 cases of cholangiocarcinoma and in 20 cases of chronic cholecystitis as control. Results (1)The positive rate of PTEN mRNA(57. 14%) and SMAD4 mRNA(61. 90%) in cholangiocarcinoma decreased significantly(P〈0.05). (2)Statistical analysis showed that there was a significant difference between the positive rates of PTEN mRNA, SMAD4 mRNA and tumor grade and prognosis (P( 0.05) respectively between the expression of PTEN mRNA, SMAD4 mRNA and cell differentiation stage or between the expression of SMAD4 mRNA and soakage and metastasis (P〈0.05). (3)There was a statistically significant positive correlation in the expression between PTEN mRNA and SMAIM mRNA(P〈0.05), a statistically significant negative correlation in the expression between PTEN mRNA and SMAD4 mRNA(P〈0. 05). Conclusion Both PTEN and SMAD4 play important roles in the development of extrahepatic cholangiocarcinoma. The detection of PTEN mRNA and SMAD4 mRNA may be useful in assessing the development of cancer,judging prognosis and eventually rendering a possible target for novel therapeutic strategies.
