两种PTCA入路治疗48例老年急性冠脉综合征合并2型糖尿病比较

摘要 目的探讨经桡动脉和股动脉穿刺介入治疗老年急性冠脉综合征(ACS)合并2型糖尿病患者的可能性和安全性。方法48例患者分别经桡动脉(A组,24例)或经股动脉(B组,24例)行经皮腔内冠状动脉成形术(PTCA)治疗,观察两组手术成功率和手术后并发症。结果两组患者接受PTCA的成功率无显著差异;局部血肿发生率(A组无,B组2例)、假性动脉瘤(A组无,B组3例)、术侧肢体浮肿(A组1例,B组2例)、动静脉瘘(A组无,B组1例)均有差异但无统计学意义,排尿困难(A组无,B组6例)、腰痛(A组无,B组18例)有显著统计学差异;两组均无远端肢体缺血。结论老年ACS伴2型DM患者经桡动脉途径行PTCA治疗术,患者痛苦小且并发症少,患者容易接受。 Objective To study the feasibility and safety of 2 PTCA approach for acute coronary syndrome (ACS) and diabetes in aged patients. Methods The PTCA was performed in 48 senile patients with ACS and diabetes. Results The PTCA approach included of per radial artery approach (group A, 24 patients) and per femoral artery approach(group B, 24 patients) respectioely. The results show no significant differences between two groups. In group B, hematoma and pseudoaneurysm were found in two cases. In group A, none of the patients suffered from pseudoaneurysm, arteriovenous fistula and ischemia of the hand. Conclusion The PTCA can be performed safely using the per radial artery approach with relatively few vascular complications and with better patient's comfort.
