摘要
目的探讨丙泊酚联合尼尔雌醇在绝经妇女取环(IUD)中的应用。方法设观察组和对照组。观察组予尼尔雌醇4mg顿服,1周后来院,术前禁饮禁食4小时,由麻醉师按丙泊酚2～3mg/kg静脉推药,待患者睫毛反射消失后施术,术中置心电监护。对照组常规宫颈注射利多卡因2ml。观察两组宫颈软化、镇痛效果、手术时间及手术效果。结果观察组宫颈软化率87.33%,完全无痛率100%,手术时间<5min者占81.33%,一次性取环成功率100%。对照组宫颈软化率36.00%,完全无痛率13.33%,手术时间<5min者占23.33%,一次性取环成功率77.33%,两组比较差异有显著性意义(P<0.01)。结论丙泊酚联合尼尔雌醇应用于绝经妇女取环,可促进宫颈软化,减少患者痛苦,提高取环成功率,值得在临床上广泛应用。
Objective To research on the application of propofol and nilestrtol in the remove operation of intrauterine device （IUD） of postmenopausal women. Methods Set up the observing group and comparative group. The observing group took the 4mg nilestriol at one time and came back to hospital one week later, who were forbidden to have angthing during four hours before operation. First the anaesthetist intravenous injected 2-3mg/kg propofol, after the disappearance of patients＇ eyelashes movement, the operation began and this process was within the monitor of electrocardiograph. The comparative group, 2ml lidocainc was injected to cervix. Then observed the softening of cervix, the analgetic effect, the operative time, the operative effect. Results In the observing group, the rate of softening of cervix was 87.33%,the rate of total painlessness was 100%,the operative time 〈5min was 81.33%, the rate of one time IUD successful remove operation was 100%. In the comparative group, the rate of softening of cervix was 36.0%,the rate of total painlessness was 13.33%,the operative time 〈5min was 23.33%, the rate of one time IUD successful remove operation was 77.33% （P〈0.01）. Conclusion The application of propofol and nilestriol in the remove operation of TUD of postmenopausal women, promoting the softening of cervix, reducing the pain of patients, in- creasing the successful rate of IUD remove operation, deserve the largo clinical application.
