摘要
目的探讨心脏瓣膜病瓣膜置换术后心脏大小的变化特点。方法对125例心脏瓣膜置换术后病人进行随访,经术前、术后7～30d平均(18.5±6.3)d、60～90d平均(70.8±7.8)d、150～180d平均(169.3±10.6)d、300～360d平均(337.4±15.2)d的胸片、心脏超声检查,对心胸比例(CTR)及左室舒张末经(LVEDD)进行回顾性分析。结果术后7￣30d首次胸片及心脏超声复查,心脏大小没有明显变化。60～90d与术前及术后第一次复查比较心脏缩小非常明显(P<0.01),心功能明显改善。术后150～180d复查与第2次比较亦有明显缩小(P<0.05)。术后300～360d心脏已无明显改变。结论心脏瓣膜病病人行瓣膜置换术后1个月内心脏大小变化不大,2、3个月内心脏缩小最为明显,心功能恢复最快,4～6个月,心脏仍有明显缩小,半年以后,心脏大小基本趋于稳定,心功能逐渐恢复。
[Objective] To investigate Postoperative change characteristics of cardiac size in patients with cardiac valve replacement. [Methods] 125 patients who underwent cardiac valve replacement were taken X-ray and echocardiography in preoperation, postoperation for 7-30 d average （18.5±6.3） d, 60-90 d average （70.8±%8） d, 150- 180 d average （169.3±10.6） d, 300-360 d average （337.4±15.2） d and retrospectively analyzed and compared the change of the cardiothoracic ration and left ventricular end--diastohc internal diameter. [ Results] After operation 7- 30 days the cardiothoracic ration and left ventricular end-diastolic internal diameter had no change apparently in the first follow up. 60-90 days after operation, the cardiothoracic ration and left ventricular end-diastolic internal diameter were compared with preoperation and the first re-examination after operation had very apparent significance （P 〈0.01）. heart function improve apparently. 150-180 days after operation, the cardiac size compared with the second re-examination had also shrink apparently （P 〈0.05）. 300-360 days after operation, the cardiac size had no change apparently. [Conclusion] The cardiac size of the patients who underwent cardiac valve replacement have no change apparently in one month, 2-3 months after operation, the heart have the most shrinking, the heart function improve apparently. 4-6 months after operation, the heart have also shrinked apprently, after six months, the heart have no shrink apparently again.
