摘要
目的探讨激光虹膜成形术治疗原发闭角型青光眼急性发作的疗效。方法对2003年3月～2004年6月确诊的原发性闭角型青光眼急性发作期患者38例42眼随机分为治疗组及对照组,对照组联合应用降眼压药物,治疗组在药物治疗的同时及时进行氩激光周边虹膜成形术,并观察眼压、前房角及预后视力。结果眼压:眼压≤21mmHg为完全控制,治疗后2小时:治疗组眼压完全控制率为85.7%;对照组为28.6%,P<0.05。治疗后6小时:治疗组为95.2%,对照组为85.7%。前房角:治疗组90.5%房角开放范围>1/2周,对照组为52.4%,P<0.05。预后视力:视力>0.5者,治疗组为62%,对照组为28.6%,P<0.05。结论激光周边虹膜成形术对原发闭角型青光眼急性发作治疗效果良好,可使功能关闭的房角迅速开放,控制眼压,从根本上改善原发闭角型青光眼的预后。
Objective To explore the effect of laser peripheral irioplasty in the treatment of the acute attack of primary angle- closure glaucoma. Methods The 38 cases （42 eyes） acute attack of primary angle - closure glaucoma that were diagnc,sed from 03, 2003 to 06, 2004 were devided at random into two groups. The control group were therapied by medicine that can decrease IOP. The therapy group were therapied by argon laser irioplastp and medicine. IOP, anterior chamber angle and visual after cure were observed. Resuits IOP≤21mmHg is whole control. The rate of whole control 2 hours after therapy is 85.7 % in therapy group. In control group the rate is 28.6% （P〈0.05）. The rate each is 95.2% and 85.7% 6 hours after therapy. Anterior chamber angle： There are 90.5% patients＇angle in therapy group open larger than 1/2 circle, but in control group the rate is 52.4% （P〈0.05）. And the rate each of patients visual after cure better than 0.5 is 62% and 28.6% （P〈0.05）. Conclusions Laser iriopasty can make better effect in acute attack of primary angle- closure glaucoma patients, h can quickly open the cloused angle, control IOP. So it can promote the therapy effect of primary angle- closure glaucoma patients.
