经颅多普勒超声检测大脑中动脉主干严重狭窄或梗阻性病变的准确性 被引量:14

The Accuracy of TCD in the Detection of Serious Stenosis or Occlusion of the Middle Cerebral Artery

摘要 目的:评价经颅多普勒超声(TCD)检测大脑中动脉(MCA)主干严重狭窄或梗阻性病变的准确性。材料和方法:对47例患MCA区脑梗死的患者进行TCD及磁共振血管造影(MRA)检查,以MRA为金标准,以收缩峰值血流速度(PSV)作为动脉狭窄的诊断参数,利用四格表法,分析TCD检测上述病变的准确性。结果:TCD对MCA中度、重度狭窄及梗阻性病变检出的敏感性、特异性、准确性、阳性预期值及阴性预期值分别为85.3%、92.3%、87.2%、96.7%、70.6%;91.7%、91.3%、91.5%、91.7%、91.3%;62.5%、100%、93.6%、100%、92.9%。与MRA完全相符率为80.9%(3847)。结论:在诊断MCA严重狭窄或梗阻性病变时,TCD与MRA有较好的一致性。 Purpose: To evaluate the accuracy of TCD in exploring the serious stenosis or occlusion of MCA. Materials and Methods: 47 patients with cerebral infarction in MCA territory underwent TCD and MRA. The peak systolic velocity(PSV) was taken as the TCD diagnostic parameter of arterial stenosis. Results: The sensitivity, specificity, accuracy, PPV and NPV of TCD in moderate, severe stenosis and occlusion of MCA were 85.3%, 92.3%, 87.2%, 96.7%, 70.6%;91.7%, 91.3%, 91.5%, 91.7%, 91.3%;and 62.5%、100%、93.6%、100%、92.9% respectively. The corresponding rate between TCD and MRA was 80.9%(38/47). Conclusion: The TCD correlated well with MRA in diagnosing serious stenotic or occlusive lesions of MCA.
