摘要
目的探讨枪伤前后猪脑功能PET显像及各重要脏器病理学的改变。方法采用81式自动步枪,口径7.62mm,560式子弹,选取成年健康家猪左后肢肌肉丰厚处,避开大血管和骨头,射击距离5m,射击后立即止血包扎,1h后行脑功能PET检查,6h后进行脑、心、肝、脾、肾和肺等脏器的病理学检查。结果枪伤前后脑功能葡萄糖代谢有明显差异(P<0.01)。枪伤后除肺脏有病理学改变外,其他重要脏器无明显病理学改变。肺组织主要表现为肺泡萎陷,含气量减少伴炎细胞浸润,肺水肿,肺泡结构不清,小血管淤血等。结论枪伤后猪脑PET功能显像代谢明显增加,肺部损伤明显,其作用机制与应激、局部周围组织炎性反应和远达效应有关。
Objective To study the changes in brain function PET imaging and pathology of important organs in pigs before and after receiving bullet wound.Methods Bullet wound was inflicted on healthy adult pigs in the back rumps away from large blood vellels and bones,using an automatic rifle and 7.62 mm-cliber bullets at a distance of 5 m.Bleeding was promptly checked,the wound was bandaged,and then the brain function was examined by PET after 1 hour.Six hours after the trauma,pathological examination of brain,heart,liver,spleen,lungs and kidneys was perfomed.Results There were significant differences in glucose metabolism of brain function PET imaging between before and after receiving bullet wound (P<0.01).After receiving wound, the important organs except lungs had no marked pathological changes;the lung tissues shoused alveolar collapse,decrease in air content accompanied by inflammatory cell infiltration,lung edema,vague alveolar structure,and blood stasis in small blood vessels.Conclusions In pigs inflicted with bullet wound, metabolism is enhanced markedly in brain function of PET imaging. Lung is injured remarkably,which is related to stress,inflammatory reaction of local tissues,and remote effect.
