摘要
目的:评价低剂量螺旋CT发现肺结节(≤10mm)的敏感性。方法:对常规剂量(130mA)螺旋CT发现肺结节(≤10mm)的2 7例病人,在征得病人的同意下,加做低剂量(30mA)螺旋CT。将≤10mm的肺结节分成两组:≤5mm和6~10mm。同期对12 0例无临床症状职工行低剂量螺旋CT筛查肺癌,根据性别及体重分别行15mA和30mA扫描。以本研究的标准,根据噪声和伪影将肺窗图像质量分为4级:0级为差;1级一般;2级良;3级优。并分别记为0~3分。结果:2 7名病人行常规剂量CT(130mA)时检出≤10mm的结节总数为6 32个,低剂量CT(30mA)时检出≤10mm的结节总数为6 0 3个,两者差异没有统计学意义(P >0 0 5 )。(7 4 % ) 4 8个结节是常规剂量CT(130mA)扫描单独发现的;而有(2 9% ) 19个结节是低剂量CT(30mA)扫描单独发现的。常规剂量CT(130mA)发现≤10mm结节的敏感性为97 1% ,发现≤5mm结节的敏感性为96 1% ,发现6~10mm结节的敏感性为98 8% ;低剂量螺旋CT(30mA)的敏感性分别为92 6 % ,90 5 %和96 3%。12 0例职工分别根据性别、体重行15mA和30mACT扫描筛查肺癌,15mA扫描CT图像肺窗评价平均分为2 31,30mA扫描为2 4 3;图像噪声明显、颗粒较粗大主要出现在主动脉弓以上肺尖区域和膈顶以下背侧区域。结论:运用30mA和130mA肺扫描对≤10mm的肺结节的敏感性没有差异。根据性别、体重分别运用15mA和30mA扫描,可用于早期肺癌的筛查。
Objective:To investigate the sensitivity of low-dose helical CT for pulmonary nodules (≤10mm) detection.Methods:As pulmonary nodules (≤10mm) were detected in 27 patients at standard-dose helical CT, we repeated low-dose helical CT after the patients' agreement. Nodules were assessed by size: the size of the nodules was measured electronically and categorized as ≤5mm and 6~10mm.This term, low-dose helical CT were performed to screen for early lung cancer in 120 asymptomatic people, scanning with 15mA or 30mA according to sex and weight. They subjectively rated the lung window images on a four-point scale (0:poor;1:fair;2:good;3: excellent) according to noise and artifacts.Results:In 27patients. 632 nodules (≤10mm) were detected at standard-dose helical CT(130mA).whereas 603 nodules were observed at low-dose helical CT(30mA). There were no statistically significant differences in the number of nodules detected at standard-or low-dose helical CT (P>0.05). 48(7.4%) nodules were observed only at standard-dose helical CT, and 19(2.9%) nodules were seen only at low-dose helical CT. The sensitivity of standard-dose helical CT (130mA) was 97.1% for all nodules(≤10mm),96.1% for nodules ≤5mm, and 98.8% for nodules 6~10mm; 92.6%,90.5%,96.3% respectively by the low-dose helical CT(30mA) scan. In 120 asymptomatic people,the lung window average score was 2.31 and 2.43 respectively by the 15mA and 30mA scan. Image noise and grain artifacts were present in lung apexes above the level of the aortic arch and lung bottom below the midriff.Conclusion:The sensitivity had no difference between low-dose helical CT(30mA) and standard-dose helical CT (130mA) for pulmonary nodules (≤10mm)detection .Screening early lung cancer was a feasible method with 15mA or 30mA according to sex and weight.
