摘要
目的 探讨脊柱骨髓瘤的影像学表现。方法 结合7例经骨髓穿刺或活检病理证实的脊 柱骨髓瘤的临床特点,回顾性分析其MRI、CT表现。结果 (1)发病部位:累及腰椎7例次,胸椎2例 次,骶尾椎2例次;(2)病灶数目:6例多骨多发,1例脊椎骨单发;(3)MRI信号:骨髓异常表现为 T1WI呈不均匀低信号,T2WI呈不均匀高信号;(4)病灶形态:7例呈多发灶状、斑片状,有椎体骨质破 坏及变形;(5)据T1WI骨髓信号减低的形态分以下类型:弥漫型、局灶型、"盐和胡椒"型,腰椎病变以 弥漫型多见;(6)病理性骨折:3例合并椎体病理性骨折。结论CT,尤其是MRI检查在骨髓瘤的诊 断和鉴别诊断中有着重要的作用。
Objective To investigate imaging features of myeloma of the spine. Methods The clinical manifestations and imaging findings (MRI、CT)in 7 patients with pathologically proved myeloma were retrospectively analyzed. Results (1 )location:All cases involved in axial hones; (2) numher:6 cases involved multiple bones,only 1 case involved solitary bone.(3)Signal intensity: The marrow infiltration were showed inhomogeneous low and high signal intensity on T1WI and T2WI respectively; (4) Shape:7 cases showed multipe focal features with vertebra destruction; (5)Types: diffuse,localized, 'salt and pepper'type. For lumbar spine,diffuse type was most common; (6) Pathological fractures:3 cases associate with pathological fractures of vertebrae. Conclusion The authors opinion were that CT examination especially MRI,played an important role in the diagnosis and differential diagnosis of myeloma of the spine.
