摘要
目的:烧伤后会形成病理性瘢痕,给患者带来功能障碍及影响美观等后遗症,围绕烧伤后的病症,在治疗方面有手术疗法、压力疗法、物理因子疗法及中医药等疗法,烧伤后的康复治疗应该是综合、协调地选择应用几种最为有效的措施。对烧伤后的各种康复医疗措施进行综述,指出目前烧伤后康复措施的发展方向。资料来源:应用计算机检索Medline1989-01/2004-05的烧伤后瘢痕的康复措施相关文章,检索词为“burn,scar,rehabilitation”,并限定文章语言种类为English。同时应用计算机检索CHKD期刊全文库1994-01/2004-10的烧伤后瘢痕的康复措施相关文章,检索词为“烧伤,瘢痕,康复”。研究对象无限制。资料选择:对资料进行初审,选取试验包括治疗组和对照组的文献,然后筛除明显非随机临床试验的研究,对剩余的文献开始查找全文,进一步判断是否为随机对照临床试验(RCT),纳入标准为RCT,无论是单盲,双盲或非盲法。资料提炼:共收集到30篇关于烧伤后康复措施的随机和非随机文献,15篇符合纳入标准。排除的15篇文献中,6篇为非随机临床试验,7篇因系重复的同一研究,2篇文献太陈旧。资料综合:13个试验包括1173例患者,分别对应用各种烧伤后瘢痕的康复措施予以评价。结论:烧伤后的康复治疗应该是综合、协调地选择应用几种最为有效的措施,如?
OBJECTIVE:The pathologic scar after burn will bring many sequelae to patients,such as dysfunction,influence on visage and so on.Based on the symptoms after burn,major treatments included surgical therapy,pressure therapy,physical agent therapy and tradition Chinese medicine(TCM) therapy,etc.,so the rehabilitative treatment of post burn should be chosen synthetically and coordinately to apply several effective managements.This paper aims to review all sorts of rehabilitation treatments after burn so as to point out the developmental direction for the rehabilitation treatment of post burn.DATA SOURCES:A computer based search of the Medline for relevant articles to rehabilitative management of post burn scar published between January 1989 and May 2005 was performed using key words of “ burn,scar,rehabilitation',with language of the articles limited to English.Meanwhile another research of China Hospital Knowledge Database(CHKD) for such articles published in Chinese language between January 1994 and October 2004 was also conducted with key words of “ burn,scar,rehabilitation'.The studying subjects were not limited.STUDY SELECTION:After primary checkup,the literatures about the trails including treatment and control groups were selected,and then the obvious non randomized clinical trials were screened.The full text of the rest articles was looked up for the next judgment of randomized controlled trial(RCT).The included articles were those about RCT,no matter whether it was single blind,double blind or non blind.DATA EXTRACTION:Totally 30 literatures about randomized and non randomized studies on post burn rehabilitative intervention were collected,of which 15 were in accordance with the inclusion criteria.The excluded 15 literatures included 6 non randomized clinical trials,7 repeated studies and 2 outdated studies.DATA SYNTHESIS:There were 1173 patients in 13 experiments.The efficacy of different rehabilitation management on scar after burn was evaluated.CONCLUSION:The rehabilitative treatment after burn should be the application of the most useful treatments which has been chosen synthetically and coordinately,such as,surgery,pressure therapy,ultrasonic treatment,silicon gel therapy and TCM method so as to reduce the physical,mental and community dysfunction and to improve the quality of life of the patients after burn.
