摘要
目的 探讨外伤性睫状体离断及其并发症的临床治疗效果。方法 我院 1993年 1月～ 2 0 0 3年 6月共收治钝挫伤所致的外伤性睫状体离断 19例 19眼 ,男 12例 ,女 7例 ,年龄 6～ 4 4岁 ,平均 2 5岁。就诊时眼压为 0～ 10mmHg(1kPa=7.5mmHg) ,平均 2 .4 2mmHg。 14例行睫状体显微缝合固定术 ,其中 3例联合巩膜环扎局部冷凝 ;4例行改良睫状体缝合固定术。 3例行保守治疗 ,2例行氩激光光凝。结果在 14例行睫状体显微缝合固定术病例中 ,1次手术成功者 9例 ;2次手术成功者 3例 ;3次手术成功者 2例。术后随访 1个月～ 8a ,平均眼压 11.2 4mmHg ,较术前有明显提高 (P =0 0 0 52 )。最后随访视力较术前有明显提高 (P =0 .0 12 )。结论 睫状体缝合固定术是有效的治疗外伤性睫状体离断的手段 ;改良的手术方法可进一步提高成功率 ;超声生物显微镜可准确诊断外伤性睫状体离断 ,对手术成功有重要帮助。
Objective To evaluate the diagnosis and treatment of traumatic cyclodialysis and its complications.Methods From January 1993 to June 2003,19 consecutive patients (19 eyes ),12 males and 7 females, with the diagnosis of traumatic cyclodialysis following ocular injuries were retrospectively evaluated.The patients'ages ranged from 6 to 44 years old(mean 25 years old). Presenting intraocular pressure ranged from 0 to 10 mmHg(1kPa=7.5mmHg). Surgical cyclopexy was performed on 14 patients,3 patients underwent combined scleral buckling and cryotherapy,four patients underwent improved surgery.Three patients underwent conservative management,2 patients underwent Argon laser photocoagulation.Results Of the 14 patients underwent surgical cyclopexy,cyclodialysis cleft was successfully closed with once operation in 9 patients, with the second operations in 3 patients,with the 3rd operations in 2 patients.Postoperative follow-up ranged from 1 month to 8 years.Postoperatively,intraocular pressure was 6 to 20 mmHg,mean 11.24mmHg,and visual acuity was significantly improved compared with preoperative visual acuity.Conclusion Surgical cyclopexy is a successful treatment for traumatic cyclodialysis;Improved surgical procedure may further increase success rate;Ultrasound biomicroscopy can accurately diagnose the presence of traumatic cyclodialyses and can aid in surgical management.
