髌骨测量参数分析及其在髌骨假体设计中的意义 被引量:8

Analysis of patellar parameters and significance in the patellar prosthesis design

摘要 目的:为髌骨假体的设计提供解剖参数.方法:在73块成人干燥髌骨标本上,对髌骨最大宽度、高度和厚度、髌骨骨床厚度、髌骨关节面高度等进行测量,对各侧测量数据用SPSS软件进行统计处理和相关分析.结果:(1)80.8%髌骨最大厚度在17~21mm内,74.0%髌骨骨床厚度在12~15 mm内,87.7%髌骨关节面高度在24~32 mm内,86.3%髌骨关节面厚度在4~8 mm内.髌骨最大厚度和髌骨骨床厚度、髌骨关节面高度和厚度的测量值分别为:(19.17±1.58)、(13.53±1.27)、(27.91±2.38)、(5.64±1.32)mm.(2)髌骨骨床厚度与髌骨厚度呈显著相关(r=0.580,P<0.01);髌骨关节面高度与髌骨高度呈显著相关(r=0.554,P<0.01);髌骨关节面厚度与髌骨骨床厚度呈负相关(r=-0.27.1,P<0 05).结论:本文测量结果可供髌骨骨床保留厚度与设计髌骨假体提供解剖参数. Objective: To provide anatomic parameters for the patellar prosthesis design. Methods: The maximal width, height and thickness of patella were measured on 73 adult patellar specimens (left 38, right 35). Besides, the bony patellar mattress thickness and other parameters were also measured on patellar specimens. All data were analyzed by SPSS software. Results: 80.82% of the parameters of patellar maximal thickness were 17-21 mm and 73.97% of the parameters of bony patellar mattress thickness were 12~15mm. 87.67% of the parameters of patellar articular surface height were 24~32mm and 86.30% of the parameters of patellar articular surface thickness were 4~8 mm. The above parameters were 19.17±1.58 mm,13.53±1.27 mm,27.91±2.38 mm and 5.64±1.32 mm respectively. Correlation between bony patellar mattress and patellar thickness was statistically significant (r=0.580, P<0.01). Correlation between patellar and patellar articular surface height was statistically significant (r=0.554, P<0.01).Correlation between patellar articular surface and bony patellar mattress thickness was statistically different (r=-0.271, P<0.05). Conclusions: The above data can provide anatomic parameters for designing patellar prosthesis and deciding the thickness of bony patellar mattress preservation.
