摘要
目的 探讨彩色多普勒超声及B Flow技术在椎基底动脉供血不全 (Veretebrobasilarinsufficiency ,VBI)诊断中的价值。方法 应用彩色多普勒 ,B Flow技术对 90例VBI患者 180条椎动脉进行检测 ,观察椎动脉形态学变化 ,并测量椎动脉内径、血流速度各参数。 5 0例健康体检者 10 0条椎动脉作为对照组。结果 90例患者 180条椎动脉中有走行扭曲 2 6条 ,管径均匀狭窄 38条 ,血管闭塞 15条 ,流速速度降低 (PSV 34.5 5± 8.5 7cm/s,EDV 9.0 8± 4 .71cm/s ,TAV14 .6 5± 5 .2 3cm/s) 10 0条 ,RI增高 (0 .76± 0 .0 7) 110条 ,各种异常椎动脉共 132条 ,检出率73% ;对照组椎动脉形态及流速异常共 14条 ,检出率 14 % ,两者比较 ,有显著性差异 (P <0 .0 1)。结论 VBI与椎动脉流速降低 ,阻力增高等血流动力学改变及走行扭曲 ,管腔狭窄等形态学改变有密切关系。彩色多普勒超声及B-Flow技术联合使用 ,可观察VBI患者椎动脉形态学上的改变 ,获取血流动力学参数 ,有助于VBI的诊断。
Objective To assess the value of color Doppler flow imaging (CDFI) and B-Flow ultrasonography in the diagnosis of vertebrobasilar insufficiency.Methods A total of 180 vertebral arteries of 90 patients with vertebrobasilar insufficiency underwent color Doppler flow imaging (CDFI) and B-Flow ultrsonography.Morphological changes of vertebral artery were studied and the diameters and blood flow parameters were measured.100 vertebral arteries of 50 health volunteers were included and served as a control group.Results Tortuosity of vertebral artery was found in 26 vertebral arteries,vascular stenosis in 38,vascular occlusion in 15,decrease of blood flow ( PVS 34.55±8.57 cn/s,EDV9.08±4.71 cm/s,TAV14.65±5.23 cm/s ) in 106,and increase of RI (0.75±0.17) in 100,acounting a total of 132 cases of abnormality in 180 vertebral arteries,the lesion detection rate being 73%.There was difference between the VBI group and the control group.Conclusion Close relation is existed between VBI and decrease of blood flow,tortuosity,and vascular stenosis of vertebral artery.The combinations of CDFI and B-Flow can give more information on the morphological changes of vertrbral artery so as to evaluate the dynamic status,and is very helrful in the diagnosis of VBI.
