加减味耳聋左慈丸对庆大霉素耳毒性的防治及其机制研究 被引量:12

摘要 目的:观察耳鸣、耳聋的传统中药方剂耳聋左慈丸加减味对庆大霉素耳毒性的防治作用,并探讨其作用机制。方法:选用耳郭反射正常,体质量为300~350g的健康杂色豚鼠,雌雄兼用,随机分成3组,对照组15只,肌肉注射生理盐水2mL/(kg·d),生理盐水4mL/(kg·d)灌胃;庆大霉素组15只,肌肉注射硫酸庆大霉素80mg/(kg·d),生理盐水4mL/(kg·d)灌胃;中药组15只,肌肉注射硫酸庆大霉素同庆大霉素组,中药煎剂浓缩液4mL/(kg·d)灌胃。各组连续用药均25d。用脑干听觉诱发电位的方法检测庆大霉素耳中毒豚鼠听阈的变化;用组织化学的方法检测耳蜗毛细胞酸性磷酸酶的变化。结果:用药前对照组、庆大霉素组和中药组的脑干听觉诱发电位反应阈值分别为:32.28±2.55,32.34±2.52和32.32±2.54;用药后第27天分别为:32.30±2.73,54.95±18.62和41.76±9.36。用药后27d庆大霉素组和中药组的脑干听觉诱发电位反应阈值升高,差异有显著性意义(t=3.466,P<0.01)。耳蜗毛细胞内酸性磷酸酶染色的变化:耳蜗铺片在光镜下观察:正常豚鼠耳蜗基底膜铺片毛细胞酸性磷酸酶染色呈棕褐色,显色分布均匀,毛细胞排列整齐;庆大霉素致耳聋的豚鼠耳蜗毛细胞酸性磷酸酶显色变化依动物耳聋程不同,出现染色缺失程度亦不同;庆大霉素组酸性磷酸酶染色缺失显著,中药组酸? AIM:To observe the preventive effects of the traditional Chinese medicine modified erlong zuoci wan for treating tinnitus and deafness on gentamicin ototoxicity,and explore its action mechanism. METHODS:Healthy mottled guinea pigs with normal auricular reflexes(300 g to 350 g in body mass, of either sex) were randomly divided into 3 groups:15 in control group were injected with saline 2 mL/kg per day intramuscularly and gastrically perfused with saline 4 mL/kg per day;15 in gentamicin group were injected with gentamicin sulfate 80 mg/kg per day intramuscularly and gastrically perfused with saline 4 mL/kg per day;15 in traditional Chinese medicine group were injected with gentamycin sulfate intramuscularly same as the gentamicin group,gastrically perfused with the traditional Chinese medicine concentrated decoction 4 mL/kg per day.All animals were administered for 25 days.The threshold of gentamicin intoxication of ears in rats was detected by brainstem auditory evoked potential(BAEP),and the changes of acid phosphatase in the cochlear hair cells were examined by histochemistry. RESULTS:The thresholds of BAEP reaction in the control group,the gentamicine group and the traditional Chinese medicine groups before administration were 32.28±2.55,32.34±2.52 and 32.32 ±2.54,respectively,and after 27 day administration were 32.30±2.73,54.95±18.62 and 41.76±6.36,respectively.There was a significant difference in the elevation of threshold of BAEP reaction between the control group and the gentamycine group after 27 day administration(t= 3.466,P< 0.01).The change of acid phosphatase in the cochlear hair cells:After acid phosphatase histochemical staining,a dark brown color could be observed with the light microscope in the surface preparation of normal guinea pigs cochlear basal membrane and the hair cells were even in color and arranged in good order.The degree of deafness and the degree of staining deletion were different when the change of acid phosphatase chemical reaction color in cochlear hair cells of guinea pigs of gentamicine induced deafness was different.There was an obvious deletion of the acid phosphatase level in the gentamicine group.There was no obvious change of acid phosphatase level in the traditional Chinese medicine group.There was a significant difference between the two groups. CONCLUSION:The modified erlong zuoci wan can reduce genetamicin ototoxicity.It protects the integrity of lysosome and reduces the autolysis of the hair cells induced by hydrolase overfloweing from lysosome,which is one of the mechanisms of reducing genetamicin ototoxicity.
