摘要
目的 :评价泌尿外科手助腹腔镜手术 (HALS)的使用情况。方法 :2 0 0 1年 11月～ 2 0 0 4年 6月 ,我院施行泌尿外科不同病种的HALS共 2 6例 ,其中肾癌 12例 ,肾盂癌 3例 ,巨大肾积水 4例 ,亲属供肾 5例 ,巨大肾上腺肿瘤 2例。肿瘤直径 3～ 8cm。手术类型为根治性肾切除、肾输尿管切除、肾切除和肾上腺肿瘤切除等。结果 :①根治性肾切除平均手术时间 12 0min ,术中出血 5 0～ 10 0ml。②肾输尿管切除平均手术时间 15 0min ,术中出血 5 0～ 10 0ml。③亲属供肾切除平均手术时间 114min ,术中出血 5 0～ 12 0ml,热缺血时间 3～ 10min ,平均 4 .4min ,1周内血肌酐均恢复正常水平。所有移植肾功能正常 ,5名受者均不需要血液透析。④巨大肾积水肾切除平均手术时间 90min ,术中出血 30～ 5 0ml。⑤巨大肾上腺肿瘤切除平均手术时间 130min ,术中出血 6 0～ 12 0ml。本组术后随访 3～ 2 8个月 ,未见肿瘤复发、转移 ,无一例中转开放手术。术后住院一般在 7天之内。手助切口 1例感染并延迟愈合。结论 :手助腹腔镜手术可以缩短手术时间 ,降低手术的危险性 ,使已有腹腔镜技术基础的泌尿外科医生迅速开展各类手术 ,亦使某些单纯腹腔镜技术难以完成的复杂手术得以完成。
Objective:To evaluate the use of hand-Assist Laparosocopy in urology.Methods:Twenty-six cases of renal cell carcinoma(12 cases),renal pelvic carcinoma(3 cases,giant nephrohydrosis(4 cases),giant adrenal tumor(2 cases)and live donor nephroectomy(5 cases)were received hand-assist laparoscopic surgery from 2001 to 2004.The diameter of tumor was from 3 to 8 cm.Results:The average operation time was 120 min in radical nephrectomy, 150 min in nephroureterectomy, 114 min in live donor nephroectomy, 90 min in nephrectomy, 130 min in adrenakectomy. No recurrence and metastasis was found in any tumor cases following 3～28 months.The renal function was normal in 5 live donor renal transplantation.The length of stay in hospital is no more than 7 days commonly.No complication of CO 2 absorption was found in any cases.Conclusions:Hand-Assist Laparoscopy is useful in urology when the incised organ should be take out integrity such as renal tumor,live donor nophroectomy and some operations which were difficult to complete with laparoscopic surgery.
