替米沙坦、福辛普利对高血压左室肥厚患者血清瘦素、一氧化氮浓度的影响 被引量:3

Effects of telmisartan and fosinopril on the levels of serum Leptin, serum nitric oxide in patients with hypertensive left ventricular hypertrophy

摘要 目的 :探讨高血压并左心室肥厚 (LVH)与血清瘦素 (Leptin)、一氧化氮 (NO)浓度的关系 ,及替米沙坦(telmisartan)、福辛普利 (fosinopril)对其影响。方法 :选取 5 8例高血压并LVH患者 (Ⅰ组 ) ,随机平均分为 2亚组 (Ⅰa、Ⅰb) ,Ⅰa、Ⅰb组分别给予 2 4周的替米沙坦、福辛普利干预 ;选取 5 8例高血压无LVH患者 (Ⅱ组 )。Ⅱ组及Ⅰ组治疗前后放免法测定血清Leptin浓度 ,硝酸还原酶法测定NO活性。结果 :Ⅰ组治疗前血清Leptin水平较Ⅱ组高 ,NO活性较Ⅱ组低 (P <0 .0 1) ;LVMI与Leptin呈正相关 ,与NO呈负相关 (r分别为 0 .2 76和 - 0 .2 6 8,P均 <0 .0 5 )。Ⅰa、Ⅰb组治疗前各指标比较差异无统计学意义 (P >0 .0 5 ) ;经 2 4周治疗后 ,Ⅰb组较Ⅰa组血清NO升高及LVMI降低水平差异有统计学意义 (P <0 .0 1)。结论 :Leptin、NO分别是促进和抑制LVH的重要物质 ;福辛普利在逆转LVH和升高血清NO方面优于替米沙坦。 Aim: To investigate the relationship between hypertensive left ventricular hypertrophy (LVH) and the lev-els of serum Leptin, serum nitric oxide (NO) as well as the effects of telmisartan and fosinopril on them .Methods:The levels of serum Leptin,serum NO in 58 cases of essential hypertensive (EH ) patients with LVH (group I ) and 58 cases of EH without LVH (group Ⅱ ) were measured. Group I was randomly divided into group Ⅰa and group Ⅰb, which took telmisartan,fosinopril for 24 weeks, respectively.Serum Leptin level was measured by radioimmunoassay and serum NO level was assayed with nitric-acid-reductase.Results:Serum Leptin level was higher in patients of group Ⅰ than that of group Ⅱ (P < 0.01) , but serum NO level was lower in group Ⅰ than that of group Ⅱ ( P< 0.01 ) . LVMI level was positively correlated with Leptin level, but negatively correlated with NO level(r=0.276 and -0.268;P<0.05). No difference was found in the levels of serum Leptin,serum NO, and the level of LVMI before treatment (P> 0.05 ). But there were significant differences in the levels of serum NO and LVMI after 24 weeks' treatment between group Ⅰa and group Ⅰb (P< 0.01) . Conclusions: serum Leptin, serum NO are important substances which act as promoting and inhibiting factors on LVH, respectively. ACEI’s effect of reducing LVH and ascending the level of serum NO is greater than ATRB’s.
