摘要
目的 分析低剂量分次照射脾脏联合放射治疗食管癌的临床疗效和失败原因。方法 1998年 8月至 2 0 0 0年 10月 60例食管癌放疗患者随机分为联合组及对照组 ,联合组放疗配合低剂量分次照射脾脏 ,对照组单纯放射治疗。临床分期 (UICC 1997TNM分期法 )联合组 :Ⅱa 8例 ,Ⅱb 13例 ,Ⅲ期 9例 ;对照组 :Ⅱa 9例 ,Ⅱb 10例 ,Ⅲ期 11例。生存分析采用SPSS12 .0软件进行Kaplan Meire分析 ,采用Log rank进行显著性检验。率的比较采用 χ2 检验。结果 联合组与对照组中位数生存期分别为 2 5个月和 18个月 ,两组 1、3、5年生存率分别为 72 .41%、43 .97%、19.84%和 80 .0 0 %、3 0 .0 0 %、10 .0 0 % ,均无统计学差异。两组死亡主要原因均为局部复发、未控和远处转移 ,无统计学意义 (P >0 .0 5 ) ,但对照组中非癌死亡为 5例 ,联合组中仅 1例死于肺部感染 ,两组间非癌死亡率有统计学差异 ,非癌死亡率联合组明显低于对照组 (P <0 .0 5 )。结论 低剂量分次照射脾脏联合放射治疗能明显降低食管癌患者的非癌死亡率 ,从而提高生存率。
Objective To evaluate the clinical effects of low dose fractionated irradiation on spleen combined with radiotherapy in patients with esophageal cancer.Methods From Aug.1998 to Oct.2000,60 patients with esophageal carcinoma were treated.They were randomly divided into combined group(30 cases)and control group(30 cases).There were 8 patients with stageⅡa,13 stage Ⅱb,9 stage Ⅲ lesions in combined group,and 9 stage Ⅱa,10 stage Ⅱb,11 stage Ⅲ lesions in control group.Survival analysis was performed by Kaplan-Meire estimates and the comparison between groups were performed by Log-rank test.Chi square test was used in the comparison of rates.Results The 1-,3-,5-year survival rates were 72.41%,43.97%,19.84% in the combined group and 80.00%,30.00%,10.00% in control group(P>0.05).The difference of residual or recurrent cancer and metastasis in both groups was similar.The motality of non-cancer was statistically significant in both groups(P<0.05).Conclusion The motality of non-cancer was decreased and 5-year survival rates coundnt be increased by low dose fractionated irradiation on spleen combined with radiotherapy for esophageal carcinoma.
