摘要
目的 :探讨超声心动图诊断胎儿心律失常的声像表现。方法 :应用彩色多普勒超声诊断仪对孕妇年龄 2 0~ 32岁、胎龄 17~ 4 0周、临床疑有胎儿心律失常或伴其他异常妊娠史的 32 5例胎儿进行超声心动图检查。通过分析超声心动图特征 ,明确房室激动顺序和时相关系 ,分类诊断胎儿心律失常。对出生后仍有心律失常的新生儿进行超声心动图与心电图同步检查验证。结果 :32 5例高危胎儿中产前超声心动图诊断胎儿心律失常 5 5例 ,其中房性早搏 2 9例 ,室性早搏 9例 ,心动过缓 7例 ,窦性心动过速 4例 ,室上性心动过速 2例 ,房室传导阻滞 2例 ,房性早搏合并窦性静止 2例。出生后仍有心律失常的 4例新生儿进行超声心动图与心电图同步检查证实结果一致。结论 :超声心动图能诊断多种类型的胎儿心律失常 。
Objective:To investigate the sonographic appearances of echocardiography on diagnosis of fetal arrhythmias. Methods:Fetal echocardiography was performed in 325 high-risk fetuses for clinically suspected. The pregnant women with 17~40 weeks gestation, aged 20~32 years were examined by ultrasound imaging system with 2.5~4.0 MHz frequency. The classification of fetal arrhythmias was identified through analysis the feature of echocardiography, determination the atrioventricular transductive sequence and relationship of time-phase. Synchronized echocardiography and electrocardiogram examination were used in the neonates who still suffered from arrhythmia. Results:Among the 325 fetuses, 55 arrhythmia fetuses were diagnosed by prenatal echocardiography. There were 29 atrial premature contraction, 9 ventricular premature contraction, 7 bradycardia, 4 sinus tachycardia, 2 supraventricular tachycardia, 2 atrioventricular block, and 2 atrial premature contraction combined sinus arrest. The results of echocardiography were consistent with those of electrocardiogram in the 4 neonates who still suffered from arrhythmia. Conclusions:Echocardiography plays an important role in the diagnosis and classification of fetal arrhythmia.
