摘要
目的 探讨激光周边虹膜成形术治疗中晚期闭角型青光眼的疗效。方法 激光周边虹膜成形术治疗 78眼中晚期青光眼 ,治疗前局部使用 2～ 3种抗青光眼药物 ,眼压仍超过2 1mmHg(1kPa=7.5mmHg) ;均有虹膜周边前粘连 ,前房角关闭 36 0° 36眼、2 70° 39眼、180° 3眼 ,使用多波长氪绿激光 ,光凝虹膜根部 ,激光参数 :能量 30 0～ 4 0 0mW ,时间 0 2s,光斑大小 2 0 0～ 30 0 μm ,光凝范围根据房角检查结果而定。 结果 术后随访 6～ 2 4个月 ,78眼中 5 6眼眼压可以控制在 2 1mmHg以下 ;6 9眼房角有不同程度的增宽或开放 ;手术前后视力、视野及眼底无明显变化 ;除有不同程度的虹膜炎反应外无其他严重并发症。结论 激光周边虹膜成形术是治疗中晚期闭角型青光眼的有效方法之一。
Objective To observe the effect of laser peripheral iridoplasty(LPI)therapy for middle and later-stage of angle-closure glaucoma.Methods Seventy eight cases of middle and later-stage of angle-closure glaucoma were treated with laser peripheral iridoplasty. Before LPI the intraocular pressure(IOP) of all cases were beyond 21mmHg(1kPa=7.5mmHg) though 2 or 3 kinds of anti-glaucoma drops were used and the anterior chamber angle were closed from 180° to 360°. Laser peripheral iridoplasty with krypton green laser and the laser paramter is power:300～400mW;time:0.2 secords;zise:200～300μm. The range of coagulate is depend on anterior chamber angle.Results Postoperatively the IOP of 56 eyes were controlled below 21mmHg(71.7%) and 69 eyes ( 88%) anterior chamber angle reopened. The vision and field have no changed .There were no severe postoperative complications except iritis reaction.Conclusion Laser peripheral iridoplasty is an ideal method of treatment for middle and later-stage of angle-closure glaucoma.
