摘要
目的:探讨乳腺良恶性肿瘤的三维彩色血流成像特点及其临床应用价值。方法:为48例乳腺良性肿瘤患者和乳腺恶性肿瘤患者使用AIOKa5500型超声仪及Tom Tec三维成像系统进行二维和三维成像检测,观察其肿瘤的形态、内部血流分布及有无穿支血流,并计算肿瘤体积(V)、内部血流(CV)及其血流与肿瘤体积比(V1)。结果:三维重建图像显示,乳腺恶性肿瘤的外形多不规则,而良性肿瘤多表现为规则的椭园形。良、恶性肿瘤的外周血流分布无明显差异,而肿瘤内部中心血流、混合血流显示率均有显著差异,尤其是穿支血流(有33例乳腺恶性肿瘤显示出穿支血流,有4例乳腺良性肿瘤显示出穿支血流)。乳腺恶性肿瘤的V值及CV值与良性肿瘤的V值及CV值相比较,差异不显著,无统计学意义(P>0.05)。恶性肿瘤的VI值明显高于良性肿瘤,差异有统计学意义(P<0.05)。结论:三维彩色血流成像能够清晰地显示乳腺肿瘤的形态及肿瘤血管的分布特点,为良恶性肿瘤的鉴别提供重要的诊断依据。
Objective: To explore the features and clinical application value of three-dimensional color Doppler flow imaging features of benign and malignant breast tumor. Method: This study selected 48 patients with benign and malignant breast tumor. All cases were confirmed by operation and pathology. Two-dimensional and three-dimensional imaging of the benign and malignant breast tumors were got by using AIOKa5500 type ultrasound and Tom Tec 3D imaging system, to observe the tumor morphology, flow distribution, with or without perforating blood flow, and to calculate the tumor volume (V), blood flow (CV), blood flow and tumor volume ratio (V1). Results: The image of three-dimensional reconstruction displayed that the appearance of the malignant breast tumor is irregular, and benign tumors showed regular ellipsoidal. Thedifference between peripheral blood flow distribution in benign and malignant tumor were not significance, But that of blood flow inside the tumor and mixed flow had significant differences, especially 33 cases of malignant tumor showed the perforating blood flow, benign tumor showed 4 cases. To compare V and CV in malignant and benign tumors, the difference was not statistically significant (P>0.05), and the VI value of malignant tumor group was significantly higher than that in benign tumor group, the difference was statistically significant (P<0.05). Conclusion: Three-dimensional color Doppler flow imaging can clearly show the characteristic morphology and tumor vascular distribution in breast tumor, and provide important diagnostic information to identify benign and malignant tumors.
