摘要
目的观察应用调脂药物加中药糖脉康颗粒治疗2型糖尿病性视网膜病变的疗效。方法回顾性系列病例研究。选择河南省商丘市第一人民医院眼科2011年1月至6月确诊为2型糖尿病合并糖尿病性视网膜病变的患者58例,按随机数字表法将患者分为观察组和对照组,每组29例,观察组给予调脂药物加糖脉康颗粒治疗,对照组仅给予常规治疗,观察患者治疗前和治疗后12个月和18个月的临床疗效和眼底病变改善情况。两组临床疗效和荧光素眼底血管造影等级资料比较采用秩和检验。结果用药后12个月,观察组患者显效10例,有效9例,无效7例,恶化3例;对照组患者显效5例,有效7例,无效10例,恶化7例。用药后18个月,观察组患者显效14例,有效10例,无效5例,恶化0例;对照组患者显效8例,有效8例,无效9例,恶化4例。用药后12个月和18个月,观察组的临床疗效均明显优于对照组(W=-1.99,-2.01;P<0.05)。观察组用药后12个月与18个月疗效比较差异亦有统计学意义(W=-2.32,P<0.05)。用药12个月和18个月后,荧光素眼底血管造影检查,显示观察组患者眼底病变改善程度优于对照组(W=-0.25,P<0.05);观察组用药后12个月与18个月的眼底病变改善程度比较,差异亦有统计学意义(W=-2.01,P<0.05)。结论调脂加糖脉康颗粒能显著改善2型糖尿病患者的眼底病变,延缓眼底微血管改变的进程。
Objective To study the therapeutic effects of the traditional Chinese medicine plus lipid modulating in the treatment of type 2 diabetic retinopathy.Methods Fifty-eight cases of type 2 diabetic retinopathy in ophthalmology department of the first people′s hospital of Shangqiu from January 2011 to June 2011 were treated.According to table of random digit all patients were divided into observation group and control group of 29 cases.The observation group was treated with the traditional Chinese medicine plus lipid modulating, the control group was given conventional therapy.Observed the therapeutic effects before treatment and after 12 months and 18 months in patients with retinopathy improve the situation.Two groups of clinical efficacy and fundus fluorescein angiography data were compared, Wilcoxon rank test was used for statistical analysis.Results After treatment 12 months, in treatment group, there were 10 cases of markedly effective, 9 cases of effective, 7 cases of no effective and 3 cases of deterioration;in control group, there were 5 cases of markedly effective, 7 cases of effective, 10 cases of no effective and 7 cases of deterioration.After 18 months, in treatment group, there were 14 cases of markedly effective,10 cases of effective, 5 cases of no effective and 0 cases of deterioration; in control group, there were 8 cases of markedly effective, 8 cases of effective, 9 cases of no effective and 4 cases of deterioration.The difference of the therapeutic effect was also statistically significant between 12 months and 18 months in treatment group (W=-2.32,P<0.05).After 12 months and 18 months, the fundus fluorescein angiography showed that the improvement of retinopathy in the treatment group was more than the degree in control group(W=-0.25,P<0.05).The difference of the improvement degree was also statistically significant between 12 months and 18 months in treatment group (W=-2.01,P<0.05).Conclusion Curative effect is the traditional Chinese medicine plus lipid modulating in the treatment of type 2 diabetic retinopathy, it can delay the progress of retinal microvascular disease.
