摘要
目的观察自体松质骨打压植骨方法修复膝关节置换术中胫骨平台骨缺损的疗效,评价该方法的应用价值。方法对50例行膝关节置换术的胫骨平台骨缺损患者术中采用自体松质骨打压植骨方法进行修复。术后均行6个月及1、2年3个时间点随访,对膝关节力线偏差及KSS评分进行统计学分析,评价近、远期疗效。结果术后影像学检查显示胫骨假体下方骨质缺损征象消失,假体位置良好,承载可靠。胫骨平台骨缺损得到有效修复。膝关节主动屈曲程度及KSS评分均明显改善,与术前比较差异有统计学意义(P<0.01)。结论膝关节置换术中自体松质骨打压植骨方法修复胫骨平台骨缺损,术中截骨得到了合理利用,具有机械力影响小,术后功能恢复完善等优势,是修复膝关节置换术中胫骨平台骨缺损的理想方法。
Objective To observe the use of autologous cancellous bone grafting method to suppress the repair knee arthroplasty tibial bong defects and long-term effect of the recent evaluation of this surgical method of application.Methods 50 routine knee arthroplasty with autogenous cancellous bone grafting method to suppress the repair of bone defects in tibial plateau in patients,collected clinical data were retrospectively analyzed all patients for 6 months,1 year and 2 years follow-up,their lines of force deviation and KSS knee score was used for statistical analysis,evaluation of the efficacy of short and long term operation.Results Tibial plateau bone defect by autogenous cancellous bone grafting method has been effectively suppressed the repair of postoperative imaging studies show lower tibial bone defect signs disappear,implant in good position,bearing and reliable follow-up shows that knee surgery flexion of the joint initiative and KSS scores were significantly improved,the difference was statistically significant(P<0.01).Conclusions The knee replacement surgery with autologous cancellous bone grafting method can be used to suppress the repair of tibial of tibial plateau bone defect.The surgery was a reasonable use of osteotomy,and can be mechanics have little impact on postoperative functional recovery advantages of being perfect,which is to repair the knee arthroplasty tibial bone defect an ideal method.
