摘要
为了探讨甘精胰岛素联合二甲双胍与格列美脲联用二甲双胍对于病程短的2型糖尿病在门诊强化血糖控制方面的疗效和安全性,将100例病程≤3年,使用二甲双胍缓释片500 mg bid超过3个月的门诊治疗的T2DM患者随机分为甘精胰岛素组(50例)和磺脲类药物组(50例),分别给予甘精胰岛素联用二甲双胍或格列美脲联用二甲双胍治疗12周,观察各组患者血糖、糖化血红蛋白(HbA1c)、C肽、血脂、血糖达标时间等指标的变化。结果显示,治疗后2组患者的空腹(FPG)及餐后2 h血糖(2hPG)、糖化血红蛋白水平(HbA1C)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)均显著下降(P<0.05～0.001);高密度脂蛋白(HDL-C)、空腹C肽(FCP)、餐后2 h C肽(2hCP)较治疗前有所增加(P<0.05);甘精胰岛素组与格列美脲组比较:FPG、2hPG、HbA1c水平显著降低(P<0.001);FCP、2hCP有所增加(P<0.05);甘精胰岛素组达标时间明显缩短(P<0.001),低血糖发生率低(P<0.01)。甘精胰岛素或格列美脲联用二甲双胍均能有效地改善T2DM患者的高血糖、高血脂状态。在控制高血糖、高血脂方面,甘精胰岛素组优于格列美脲组,达标时间更短、安全性更高。
Objective:To explore the safety and effect of strengthening control of blood glucose with insulin giargine plus metformin or glimepirde plus metformin for short duration of type 2 diabetes in outpatient.Methods:100 outpatient of type 2 diabetes(duration ≤ 3years) with metformin(500 mg bid) more than three months were studied then with insulin glargine(n=50) or glimepirde(n=50) for 3 months.Bloodglucose,blood lipid,glycatedhemoglobin,C-peptide,glucose standard time changes were observed in the two groups.Results:The preprandial blood glucose(FPG),2-hour postpraendial blood glucose(2hPG),glycosylated hemoglobin(HbA1C) and Total cholesterol(TC)、Triglycerides(TG),low density lipoprotein(LDL-C),were significantly reduced after 3-month treatment in both groups as compared with those at baseline(P< 0.05～0.001),but insulin glargine plus metformin reduced more obviously(P<0.001);high density lipoprotein(HDL-C)、fasting C peptide(FCP)and 2-hour postpandial C peptide(2hcp) increased(P<0.05),insulin glargine plus metformin increased more obviously(P<0.05);The group with insulin glargine plus metformin significantly shortened the time(P<0.001),incidence of hypoglycemia was low(P<0.01).Conclusion:Insulin Glargine or glimepirde plus metformin were effective in improving T2DM patients' symptom of hyperlipidemics and hyperglycemia.As to the control of hyperlipidemics and hyperglycemia,the group with insulin glargine can shorten the achieved time and be much safer.
