摘要
本文对23例椎管内神经鞘瘤的 MR 信号及病理所见进行了分析和对比。该肿瘤在 T_2加权序列(SE—T_2WI 或 RARE—T_2WI)中皆呈脑脊液样强 MR 信号;在 SE—T_1WI 中多数肿瘤(73.9%)信号与脊髓相近,少数(26.1%)信号低于脊髓.经与手术病理对照研究,发现肿瘤呈长 T_2值的特性除一般文献报道的与肿瘤内囊变有关外,更重要的是与肿瘤内含有 Antoni B 型结构有关。另外,肿瘤内坏死灶也是引起 T_2值延长的因素之一,并指出,肿瘤内血管畸形可能也参与了高信号的形成。肿瘤 T_1值多数与脊髓相近缘由肿瘤内含有较为丰富的雪旺氏梭形细胞,同时也与肿瘤内含有的微小囊变、Antoni B 型结构等生化成份有关.
The results of MR signal intensities and pathological findings were correl- atively described in 23 cases of spinal neurinomas.With T_2-weighted imaging(SE-T_2WI or RARE-T_2WI),all the tumors were displayed as bright signal intensities similar to that CSF,indicating that neurinomas had very long T_2 values On SE-T_1WI,the majority of neurinomas(73.9%)had equal and the others(26.1%)low signal intensities compared to that of the spinal cord.Correlating to pathological findings,it was known that long T_2 val- ues of the neurinomas were caused not only by intratumoral cystic formation that was re- ported by other authors,but also related to the following factors:(1)intratumoral Antoni B type of structure,a very important factor,which existed in all the tumors;(2)intratumoral micro—necrosis which was found in 17.4% of the tumors;(3)intratumoral vasular malfor- mation which was seen in 30.4% of the neurinomas.The T_1 values of neurinomas which were similar to that of the spinal cord in most of the tumors may be related to rich Schwann's cells within the tumors and also to biochemical composition of the tumors.
