应用C-L短柱瓣行瓣膜替换术后早期血液动力学的变化 被引量:9

摘要 目的:评价C-L短柱瓣植入人体后的血液动力学性能。方法:慢性风湿性心脏病患者128例随机分成3组,分别采用C-L短柱瓣、StJude双叶瓣、Medtronic-Hal单叶瓣作心脏瓣膜替换术,比较3种瓣膜植入人体后72h内血液动力学的变化。结果:应用C-L短柱瓣组,其术后的血液动力学变化趋势与应用StJude组或Medtronic-Hal组相仿,在术后24h后均能显著改善患者的血液动力学(P<0.05),3组间无统计学差异(P>0.05);而且术后早期3组血管活性药物的使用亦无明显差异(P>0.05)。结论:C-L短柱瓣植入人体内,对血液动力学的影响与目前已公认优良的人造心脏瓣膜相比无明显差异。 Objectives: To evaluate the hemodynamic performance of C L pugestrut heart valvular prosthesis in patients undergoing heart valve replacement. Methods: One hundred and twenty eight patients with rheumatic heart disease (RHD) were divided randomly into three groups: C L group with the implantation of C L pugestrut valve, St Jude group with St Jude bileaflet valve and Medtronic group with Medtronic Hall valve. Results: Mean pulmonary arterial pressure, pulmonary arterial wedge pressure, systemic vascular resistance index and pulmonary vascular resistance index were significantly reduced immediately after heart valve replacement in three groups while cardiac index, stroke index, left cardiac work index, left ventricular stroke work index, right cardiac work index and right ventricular stroke work index were significantly improved (P<0.05) in all three groups after 24 h postoperatively. There were no statistical differences among the three groups in regarding to the above hemodynamic parameters as well as the postoperative usage of administered vasoactive agents(P>0.05).Conclusion: C L pugestrut valve implanted in vivo results in an excellent hemodynamics in patients undergoing cardiac valve replacement.
