摘要
目的:探讨输尿管镜下钬激光联合Ntrap结石拦截网治疗输尿管上段结石的临床治疗效果.方法:2009年6月～2011年lO月收治输尿管上段结石76例,均行输尿管镜下钬激光碎石治疗,术中置入Ntrap结石拦截网以阻断结石上移.对手术时间、碎石成功率及输尿管有无穿孔等情况进行分析.结果:一次性碎石成功70例,6例结石上移入肾脏,术后结合ESWL治疗,碎石成功率(92.1%),术中无输尿管穿孔、黏膜撕脱等并发症,手术时间(504±12)min,术后行KUB及B超检查均无结石残留.结论:输尿管镜下钬激光联合Ntrap结石拦截网治疗输尿管上段结石能有效防止碎石过程中结石上移,可明显提高碎石成功率.
Objective:To evaluate the clinical efficacy of Ntrap stone extraction combined with ureteroscopic holmium laser lithotripsy for the treatment of upper ureteral stones.Methods:Seventy-six patients of upper ureteral stones underwent ureteroscopic lithotripsy using holmium laser with Ntrap stone extraction by a rigid ureteroscopy in our hospital between June 2009 and October 2011.The success rate of lithotripsy,operation time,rate of ureteral perforation,clearance of calculus were retrospectively analyzed.Results:The procedure was completed successfully in all the cases,and in 92.1%(70/76)of them,the stones were fragmented in one session.In 6cases,the stones were pushed upward and thus the patients received extracorporeal shock wave lithotripsy(ESWL)afterwards.None of the 76 patients had ureter perforation.The mean operation time was(50±12)min.No lithiasis fragments were observed by the post operation examination of KUB and ultrasonography.Conclusions:Ntrap stone extraction can effectively prevent ureteral stones from moving upward during ureteroscopic holmium laser lithotripsy.Combined with holmium laser lithotripsy,it is effective,safe and minimally invasive for upper ureteral stones.
