摘要
目的分析镰旁脑膜瘤的MRI征象与病理,以期提高其临床诊治水平。方法 73例镰旁脑膜瘤患者行MRI检查。将位置、大小、形状、边缘、水肿、信号、强化方式等影像特征、病理特点综合分析。结果除宽基底、脑膜尾征、假包膜等一般脑膜瘤的MRI表现外,还发现了如下不常见MRI表现:囊变2例,出血2例,钙化5例,颅骨受侵4例及肿块跨越征2例,病理可见相应表现。结论大脑镰旁脑膜瘤具有囊变、出血、钙化、颅骨受侵及肿块跨越征等不常见MRI征象。熟悉这些不典型表现与手术、病理综合分析,在肿瘤的诊断、鉴别诊断与治疗均具有很大价值。
Objective To analysis the relationship between the MRI features and its pathology of parasagittal meningiomas and improve the ability of the diagnosis and treatment of it.Methods 73 cases of parasagittal meningiomas were examed by MRI.The MRI features including location,size,shape,edge,edema,signals,features of enhancement and so on were analyzed combining with pathological characters.Results Besides common MRI findings of meningiomas such as wide base,dural tail sign and false capsule,the following imaging signs were seen:cystic changes in 2 cases,hemorrhage in 2 cases,calcify in 5 cases,extra-cranial involvement in 4 cases and striding across the superior sagittal sinus and/or falx of the tumor in 2 cases,which were relevent with the results of pathology.Conclusion There are some uncommen features of parasagittal meningiomas including cystic changes,hemorrhage,calcify,extra-cranial involvement and striding sign of the tumor.Being familiar with the untipical manifestations and a combination of pathology and surgery are of great value in the diagnosis,differential diagnosis,treatment and prognosis of tumor.
