摘要
目的 探讨一种新的肺标本制作方法 -气液双重灌注固定法 ,以期较快地获得更接近肺固有形态的充气肺标本。材料与方法 收集 2 0个手术切除的新鲜肺叶标本 ,分为两组 ,A组 13个 ,用气液双重灌注法固定 ;B组 7个 ,用Heitz man法固定。术前及固定后的肺标本均行HRCT扫描。对标本外形、大体切面及组织学切片的质量分为三级进行评估。对肺叶标本及术前的HRCT图像进行对照分析。结果 标本平均干燥时间A组约为B组的一半。标本质量 :A组 ,Ⅰ级 10个(10 / 13,76 .9% ) ,Ⅱ级 3个 (3/ 13,2 3.1% ) ;B组 ,Ⅱ级 3个 (3/ 7,42 .9% ) ,Ⅲ级 4个 (4/ 7,5 7.1% )。标本质量与制作方法有明显相关性 (P <0 .0 1) ,A组标本质量好于B组 ,A组HRCT图像与术前较一致。结论 气液双重灌注制取的肺标本更好地保留了肺的固有形态 ,有助于更精确地进行肺HRCT的解剖学与病理学研究。
Purpose To explore a new method of preparing inflated lung specimen,the double gas-liquid perfusion fixation,in order to obtain inflated specimen retaining the original lung structures.Materials and methods20 fresh lung lobes were collected from surgical resection and divided into two groups.In group A,13 lobes were fixed using double gas-liquid perfusion method.In group B,7 lobes were fixed using Heitzman method.HRCT scans were made before operation and for all fixed specimens.The quality of the fixed specimens were evaluated and graded into three degrees.The HRCT images of fixed specimens were compared with the[CM(50] preoperative scans.Results Mean drying time of specimens in group A was about the half in group B.In group A,10 specimens (10/13,76.9%)were considered to have grade Ⅰ quality,3(3/13,23.1%)were grade Ⅱ.In group B,3(3/7,42.9%)were gradg Ⅱ,4(4/7,57.1%)were grade Ⅲ.The quality of the specimens correlated with the fixed method(P<0.01)。The quality of fixed specimens in group A was better than in group B.The HRCT images of specimens in group A were resemblance to the preoperative HRCT.Conclusion The specimens fixed by double gas-liquid perfusion method kept the original structures of the lung,provided realistic materials for the study of HRCT related to anatomy and pathology.
