摘要
目的比较单纯使用LMWH和联合IPC预防人工髋膝关节置换术后DVT形成的效果和安全性。方法2004年1月-2008年12月,74例人工髋膝关节置换手术病例,随机分成2组。对照组36例,术后8h开始单纯皮下注射低分子肝素(速碧林),治疗组38例,术后马上开始使用间歇充气加压装置及术后8h开始使用速碧林。进行DVT症状评估、下肢深静脉的彩超检查,观察出血并发症与记录围术期出血量。结果对照组DVT8例(22.2%),出血量(814.3±312.4)ml;治疗组DVT2例(2.6%),出血量(852.2±295.4)ml。结论术后使用低分子肝素联合间歇充气加压装置预防人工髋膝关节置换术后下肢深静脉血栓效果好,安全性高。
Objective To evaluate the efficacy and safety of low molecular weight heparin(LMWH)combined with intermittent pneumatic compression in prophylaxis of postoperative deep vein thrombosis (DVT)following hip and knee surgery. Methods From January 2004 to December 2008,74 patients undergoing hip and knee orthopedic procedures were randomized into 2 group for studying. One group only was received a low molecular weight heparin with 0.3ml from the first day to 10th day and the other group was subcutaneously received low molecular weight heparin in combination with intermittent pneumatic compression following implant arthroplasty. All patients were examined by color duplex ultrasonography,valued the clinical symptom of DVT,recorded the volume of blood loss in perioperation and observed complication of hemorrhage.Results Eight patients (22.2%)developed DVT in the control group of 36 patients and one patients (2.6%)in the experimental group of 38 patients (P<0.05).The volume of blood loss in perioperation of two groups was no statistical significance (P>0.05).Two groups had no any bleeding complications. Conclusion The low molecular weight heparin in combination with intermittent pneumatic compression is safe and effective in preventing postoperative deep vein thrombosis in patients following hip and knee surgery.
