经治肺结核的X线平片与螺旋CT的对照研究

摘要 目的评价X线胸片与螺旋CT在经治肺结核病灶分布与活动性判断上的价值。方法收集经治1年以上的肺结核100例的X线胸片和螺旋CT检查结果,并分析肺结核病灶分布、形态及病灶活动性方面的差异。结果经治肺结核胸片上病灶主要累及二叶或二叶以下74%、CT上累及二叶为44%;累及三叶或三叶以上者为54%。病变形态上胸片主要表现为纤维化病灶(51%)和斑片状影(50%);CT主要表现为小叶中心病变(70%)、纤维化病灶(60%),斑片状病灶(45%)和淋巴结肿大(32%),空洞25例(25%),树芽征20例(20%)。结论经治1年以上肺结核X线胸片与CT对病灶分布及形态的显示上有显著差异,CT检查明显优于X线胸片。 Objective To evaluate the value in tile focus distribution and activity of the pulmonary tuberculosis after antituberculous chemotherapy.Methods The results of X-ray film and spiral CT in 100 cases above one year after antituberculous chemotherapy were studied prosperctively,the difference in the focus distribution,shape and activity of the pulmonary tuberculosis were analysed.Results Lesion after treatment mainly involved two lobe or below in the X-ray film (74% ),focus mainly involved two lobe(44% ),three lobe or above (54% ) in spiral CT scan.Comparing with the presence and the shape of focus in X-ray film and spiral CT scan.fibrous focus (51% )and patchy shadow (50%) were main manifestation in the chest film.the presence of centrilobular lesions (70%),fibrous focus appearence (60% ),patchy shadow (45% ),lymphopathy(32% ),Cavity focus (25% ),tree-in-bud appearance(20% ) in the spiral CT scan.Conclusion There was significant difference between the presence the focus distribution and shape of the pulmonary tuberculosis after antituberculous chemotherapy above one year.CT scan is obvious excellent than X-ray film.
