摘要
目的探讨超声显像在乳腺癌诊断及筛查中的应用价值。方法对我院144例病理证实为乳腺癌患者的术前超声检查资料进行回顾性分析,观察分析包括肿块大小、边界、形态、回声特点、有无钙化、后方回声增强或衰减情况,彩色多普勒探测肿块血流供应,血管形状或分布,测量血流速度、阻力指数。对有些难以鉴别(位于乳腺后缘)的肿块,触诊体会肿块活动度及硬度,并行超声引导下活检病理检查。结果144例乳腺癌中超声显示为低回声117例,内部出现钙化24例,后方回声衰减105例,肿块内显示明显血流信号62例,阻力指数0.7以上62例,平均血流速度24.99cm/s。21例肿块小于1cm,血流显示不丰富,超声图像难以鉴别良恶性。11例肿块位于乳腺后缘,触诊时活动度差,质较硬,10例位于乳房中部或前缘,活动度大。110例在超声引导下活检证实为乳腺癌。结论乳腺癌超声显像具有特征性表现,但超声显像诊断早期乳腺癌有一定难度,需在超声引导下活检病理检查明确诊断。
ObjectiveTo explore the improvement of accuracy rate in screening or diagnosing the mammary cancer on ultrasonography.MethodsPreoperative sonography in 144 patients with mammary cancer comfirmed by pathology were analyzed retrospectively. The magnitude,boundary,morphology echoes, calcification,the echoes behind the tumor, and the quantity,shape,distribution, speed resistance index of blood vessels were all analysed. When the tumor lay on the posterior edge of the breast, it was difficult to diagnose, we detected it by palpation to realize it's mobility, rigidity and large core biopsy was preformed if necessary.ResultsAmong the 144 patients 117 patients' lumps had low echoes and, 24 had calcified dots; the behind echoes were attenuated in 105 patients'; abundant blood flow appeared in 62 patients. Resistance index in 62 patients were ≥0.7. The average blood velocity was 24.99 cm/s. The nature of 21 patients' lumps less than 1cm in diameters without abundant blood flow was hardly determined on ultrasonography. Eleven patients(11/21) with hard masses difficult to move, lay on the posterior edge of the breast. Ten out of 21 patients, with lumps easy to move, lay in the middle or on the anterior edge of the breast. Under the ultrasound guidance, 110 out of 144 patients underwent large core biopsy, and were diagnosed as malignant cancer. ConclusionsMammary cancer has it's ultrasonographic features, but there is some difficult in diagnosing early mammary cancer. In order to define the diagnosis further, large core biopsy guided by ultrasound is needed.
