摘要
目的探讨脊髓损伤伴有不全瘫的患者发生腹股沟嵌顿疝的围手术期护理方式选择,并就术后疗效分析做总结及评价。方法回顾性分析2017年4月至2020年10月,北京朝阳医院急诊科收治的29例脊髓损伤后并发腹股沟嵌顿疝患者的临床资料,对患者的一般状况、手术方式选择及围手术期护理情况加以分析。结果本组29例患者均行腹股沟疝修补手术,根据患者病情选择腹腔镜或开放腹股沟疝修补术,所有患者均治愈出院,无死亡病例。术后28例患者获访,随访时间为6~12个月(平均10个月)。所有患者术后均恢复良好,无伤口感染、下肢深静脉血栓、慢性疼痛等并发症发生。结论脊髓损伤伴有不全瘫的患者发生腹股沟嵌顿疝后均应尽早手术,围手术期护理策略与单纯腹股沟疝术后护理方案不尽相同,在患者不能早期下地自由活动的情况下使用被动活动等个体化护理方案配合早期康复治疗及功能锻炼,可以获得比较满意的疗效。
Objective To explore the selection of perioperative nursing methods for patients with spinal cord injury accompanied by incomplete paralysis who had incarcerated inguinal hernia,and summarize and evaluate the postoperative efficacy.Methods Retrospective analysis of the clinical data of 29 cases with incarcerated inguinal hernia after spinal cord injury who were admitted to the Emergency Department of Beijing Chao Yang Hospital from April 2017 to October 2020.The general condition,surgical method selection,and perioperative care of the patients were analyzed.Results 29 cases underwent inguinal hernia repair surgery,and laparoscopic or open inguinal hernia repair surgery was chosen based on the patient's condition.All patients were cured and discharged without any death.28 cases were followed up for 6-12 months(average 10 months)after surgery.All cases recovered well after operation,without wound infection,deep vein thrombosis,chronic pain and other complications.Conclusion Patients with spinal cord injury accompanied by incomplete paralysis who had incarcerated inguinal hernia should undergo surgery as soon as possible.The perioperative nursing strategy is different from the postoperative nursing plan for simple inguinal hernia.In the case that patients cannot move freely in the early stage,individualized nursing plans such as passive mobility,combined with early rehabilitation treatment and functional exercise,can achieve satisfactory results.
