固定与活动平台假体在全膝关节置换术中的应用价值

Application value of fixed and mobile platform prostheses in total knee arthroplasty

摘要 目的比较固定平台假体与活动平台假体在全膝关节置换术(TKA)中应用的早期疗效区别。方法回顾性选取2019年6月至2021年6月在北京市房山区中医院骨伤科已行TKA的患者85例,根据术中应用平台假体的不同分为固定组(n=40)和活动组(n=45),采用重复测量方差和t检验分别采取固定平台假体、活动平台假体。统计两组围手术期指标、术后6个月内并发症发生情况,比较两组术前、术后3、6个月的美国纽约特种外科医院膝关节评分(HSS)、视觉模拟评分法(VAS)及步态时空参数。结果两组术中出血量、手术时间、住院时间、衬垫脱位率、并发症发生率、HSS评分比较差异无统计学意义(P>0.05)。不同时间点下固定组跨步长、步速及患侧单支撑相高于活动组,差异有统计学意义(F=21.203、9.282、20.388,均为P<0.05);术后3个月活动组跨步长、步速、患侧单支撑相高于固定组(t=3.904、14.725、7.756,均为P<0.05)。结论活动平台假体及固定平台假体均能帮助患者获得步态改善,取得让人满意的手术效果,应用活动平台假体的TKA患者术后步态改善更快,能更好地促进早期锻炼及膝关节功能的康复。 Objective To compare the early effects of fixed platform prosthesis and mobile platform prosthesis in total knee Joint replacement(TKA).Methods Eighty five patients who had been treated with TKA in the Department of Orthopaedics and Traumatology of Fangshan District Hospital of Traditional Chinese Medicine from June 2019 to June 2021 were retrospectively selected.They were divided into fixed platform prosthesis group(n=40)and mobile platform prosthesis group(n=45)according to the different applications of platform prosthesis during surgery.Perioperative indicators and the incidence of complications within six months after surgery were collected.the knee joint score(HSS),visual analogue score(VAS),and gait spatiotemporal parameters of the New York Special Surgery Hospital before surgery and three and six months after surgery were comparee between the two groups.Results There was no statistically significant difference in intraoperative bleeding volume,surgical time,hospital stay,pad dislocation rate,and complication incidence between the two groups(P>0.05).At different time points,the stride length,stride speed,and single support phase of the affected side in the fixed group were higher than those in the active group,with statistically significant differences(F=21.203,9.282,20.388,all P<0.05).After three months of surgery,the stride length,stride speed,and single support phase of the affected side in the active group were higher than those in the fixed group(t=3.904,14.725,7.756,all P<0.05).Conclusions Both mobile and fixed platform prostheses can achieve gait improvement and satisfactory surgical results.Mobile platform prostheses in TKA may provide faster postoperative gait improvement,which can better promote early exercise and knee joint function recovery.
