摘要
目的分析改良Hayman缝合术治疗剖宫产产后出血的临床效果。方法前瞻性随机对照研究。抽取2019年7月至2021年7月于郑州大学附属郑州中心医院行剖宫产术出现产后出血实施子宫压迫缝合术的患者82例,按照奇偶数分配法将其分为观察组(行改良Hayman缝合术)和对照组(行B-Lynch缝合术),每组41例。比较两组手术有效率、手术时间、术中出血量、术后24 h出血量、术后住院时间、术后重症加强护理病房(ICU)入住率、术前术后血红蛋白(Hb)下降情况,在随访中记录并比较术后产褥病发病率、血性恶露持续时间、月经复潮时间。结果观察组手术时间、术中出血量、术后24 h出血量、术前术后Hb下降程度、术后产褥病发病率少于或低于对照组(U/t/χ^(2)=7.82、24.74、7.79、5.93、5.75,P均<0.05)。两组手术有效率、术后ICU入住率、术后住院时间、血性恶露持续时间、月经复潮时间比较,差异未见统计学意义(P>0.05)。结论改良Hayman缝合术治疗剖宫产产后出血手术时间少,可减少术中及术后出血量,降低产褥病发病率,减轻术前术后血红蛋白下降程度。
Objective To analyze the clinical effect of modified Hayman suture on postpartum hemorrhage after cesarean section.Methods This was a prospective randomized controlled study.Eighty-two patients who underwent cesarean section in Zhengzhou Central Hospital Affiliated to Zhengzhou University and received uterine compression suture for postpartum hemorrhage from July 2019 to July 2021 were selected.The patients were divided into observation group(treated by modified Hayman suture)and control group(treated by B-Lynch suture)according to odd-even distribution method,with 41 cases in each group.The operation efficiency,operation time,intraoperative blood loss,24-hour postoperative blood loss,postoperative hospital stay,postoperative intensive care unit(ICU)occupancy rate,change of preoperative and postoperative hemoglobin(Hb)were compared between the two groups.During the follow-up,the incidence of postoperative puerperal disease,the duration of bloody lochia,and the time of menstrual resurgence of the two groups were recorded and compared.Results The operation time,intraoperative blood loss,24-hour postoperative blood loss,change of preoperative and postoperative Hb,and incidence of postoperative puerperal disease in the observation group were less or lower than those in the control group(U/t/χ^(2)=7.82,24.74,7.79,5.93,5.75;all P<0.05).There was no significant difference in the effective rate of surgery,postoperative ICU occupancy rate,postoperative hospital stay,duration of bloody lochia,and time of menstrual refluctuation between the two groups(P>0.05).Conclusions Modified Hayman suture in the treatment of postpartum hemorrhage after cesarean section takes less operation time,and it can reduce intraoperative and postoperative blood loss,reduce the incidence of puerperal disease,and reduce the degree of preoperative and postoperative hemoglobin decline.
