臂丛神经阻滞联合喉罩全麻用于肱骨内固定术的效果被引量：2

摘要目的评价臂丛神经阻滞联合喉罩全麻用于肱骨内固定术的效果和安全性。方法 ASA分级玉~芋级行肱骨内固定术的患者40例,随机分成两组(n=20),玉组在气管插管全麻下行静吸复合全麻,Ⅱ组先行术侧肌间沟臂丛神经阻滞,然后在喉罩下行静吸复合全麻。术后用布托菲诺行自控静脉镇痛。记录手术结束至自主呼吸恢复时间、睁眼时间、拔气管导管(玉组)或喉罩(Ⅱ组)时间、麻醉恢复室停留时间;记录麻醉前(T1)、麻醉诱导后(T2)、气管插管(玉组)或置入喉罩(Ⅱ组)即刻(T3)、手术进行30 min时(T4)、术毕(T5)、拔气管导管(玉组)或喉罩(Ⅱ组)即刻(T6)的SBP、DBP、HR;记录术后24 h内的PCA按压次数、PCA药物用量及不良反应,评价术后镇痛满意。结果与玉组比较,Ⅱ组患者术中血流动力学波动小,麻醉苏醒和恢复时间缩短,术后镇痛用药量减少,效果更满意,不良反应少。结论臂丛神经阻滞联合喉罩全麻用于肱骨内固定术麻醉效果好,苏醒与恢复快,术后镇痛效果好,不良反应少,能提高手术及麻醉的安全性。
