摘要
目的 探讨人格、主观应激对肠易激综合征(IBSSS)患者心理弹性的影响.方法 采用横断面现况调查方法,纳入2017年1月至2018年6月就诊的IBS患者.收集患者文化程度、收入、病程、肠易激严重程度评分(IBSSS)、IBS临床症状分型资料,心理弹性评估采用Connor-Davidso心理弹性量表(CD-RISC),主观应激评估采用主观应激量表(PSS),人格评估采用艾森克人格问卷(EPQ).对心理弹性影响因素进行单因素相关分析,对有统计学意义的因素进行多因素logistic回归分析.结果 本研究共入组108例.单因素相关分析结果提示,心理弹性与主观应激、肠易激严重程度评分、神经质呈负相关,与人格外向、经济收入、文化程度呈正相关.多因素相关分析提示心理弹性与以下因素存在关联:主观应激(OR=1.036)、外向性(OR=1.487)、肠易激严重程度评分(OR=0.901)、经济收入(OR=6.405).结论 IBS患者的心理弹性不仅与临床疾病病情相关,还与患者的人格、主观应激和经济收入均有关.
Objective To study relevant factors that influence psychological resilience in patients with irritable bowel syndrome(IBS),especially the role of personality and perceived stree.Methods IBS were recruited from January 2016 to June 2018 in the Department of Gastroenterology in the First Hospital of Nanchang University.The education levels,income,duration of disease,irritable bowel severity scoring score and disease phenotype were collected.Connor-Davidso Resilience Scale(CD-RISC)was used to assess psychological resilience.Subjective Stress Scale(PSS)was used to assess subjective stress.Eysenck Personality Questionnaire(EPQ)was used to assess personality.Univariate analyses were conducted to determine the correlation of variables with resilence and thereafter those statistically significant were reanalyzed via a multivariate regression model.Results A total of 108 patients with IBS were finally recruited.Univariate analyses demonstrated resilience was inversely associated with perceived stess,irritable bowel severity scoring score and neuroticism.Extraversion,income,college education were positively related to resilience.However,multivariate analyses revealed that perceived stress(OR=1.036),extraversion(OR=1.487),irritable bowel severity scoring score(OR=0.901)and income(OR=6.405)were significantly related to resilience.Conclusion Resilience of IBS patients is not only associated with disease activity,but also with personality,perceived stress and income.
