摘要
目的:分析探讨换血疗法对新生儿高胆红素血症血液学指标的影响.方法:对20例新生儿高胆红素血症患儿采取外周动静脉双通道同步换血治疗,并抽取患儿换血前后的血标本,监测血常规、生化等相关指标,并对换血前后各项指标的变化进行比较.结果:与换血前比较,换血后血清总胆红素和间接胆红素明显下降,血糖明显升高,白细胞和血小板明显降低,换血治疗前后差异有统计学意义;血清钾、钠、氯、钙无明显变化,红细胞和血红蛋白无明显变化,换血治疗前后差异无统计学意义.结论:换血疗法治疗和改善新生儿高胆红素血症效果显著,但会造成新生儿内环境的波动和变化,存在血糖紊乱、术后感染及继发性出血的风险,在换血前后密切监测血常规和血生化等血液学指标的改变,为临床提供可靠数据,以保证换血疗法的安全性.
Objective To study the effect of exchange transfusion therapy on the hematological parameters of neonatal hyperbilirubinemia.Methods 20 cases of neonatal hyperbilirubinemia in children with peripheral arteriovenous dual-channel simultaneous exchange transfusion treatment,and blood samples before and after transfusion of blood samples,monitoring of blood,biochemical and other related indicators,and before and after the exchange of the indicators Change the comparison.Results Compared with the prior exchange transfusion,exchange transfusion after serum total bilirubin and indirect bilirubin decreased significantly,blood glucose was significantly increased,white blood cells and platelets significantly decreased,before and after transfusion treatment,there were significant differences between the two groups Statistical significance;Serum potassium,sodium,chloride,calcium no significant change,no significant change in red blood cell and hemoglobin,there was no significant difference before and after exchange transfusion.Conclusions Exchange transfusion therapy and improve neonatal hyperbilirubinemia effect is significant,but will cause changes in neonatal environment and changes in the existence of blood glucose disorders,postoperative infection and secondary bleeding risk,In blood transfusion before and after close monitoring of blood and blood biochemical changes in hematological indicators,to provide reliable clinical data to ensure the safety of exchange transfusion therapy.
