摘要
目的探析早期系统康复联合髓内钉治疗肱骨近端骨折的疗效.方法选取该院2017年8月—2019年8月收治的90例肱骨近端骨折患者,根据随机分组法分为两组,每组45例.两组患者均接受髓内钉固定术治疗,在此基础上对照组采用常规康复治疗,观察组采用早期系统康复治疗.对比两组患者康复治疗后的肩关节疼痛视觉模拟评分(visual analogue score,VAS)、肩关节活动度、Contant-Murley肩关节功能评分、骨折愈合时间及不良反应发生情况.结果观察组康复训练后的VAS评分为(1.35±0.65)分,低于对照组的(1.75±0.55)分,差异有统计学意义(P<0.05).观察组康复训练后的各项肩关节活动度均大于对照组,组间差异有统计学意义(P<0.05).观察组康复训练后的Contant-Murley肩关节功能评分高于对照组,且骨折愈合时间短于对照组,组间差异有统计学意义(P<0.05).两组患者均未发生任何不良反应.结论早期系统康复联合髓内钉治疗肱骨近端骨折的疗效理想,可有效促进患者的骨折愈合及肩关节功能恢复,具有较高的安全性,值得临床应用.
Objective To explore the efficacy of early systemic rehabilitation combined with intramedullary nailing in the treatment of proximal humeral fractures.Methods 90 patients with proximal humerus fractures admitted to the hospital from August 2017 to August 2019 were selected.They were divided into two groups according to the randomized grouping method,with 45 cases in each group.Patients in both groups were treated with intramedullary nail fixation,on the basis of which the control group adopted conventional rehabilitation treatment and the observation group adopted early systematic rehabilitation treatment.The shoulder pain visual simulation score(VAS),shoulder movement,Contant-Murray shoulder function score,fracture healing time and adverse reactions were compared after rehabilitation in two groups of patients.Results The VAS score after rehabilitation training in the observation group was(1.35±0.65)points,which was lower than the control group's(1.75±0.55)points,and the difference was statistically significant(P<0.05).After the rehabilitation training of the observation group,the activities of the shoulder joints were greater than those of the control group,and the differences between the groups were statistically significant(P<0.05).The Contant-Murray shoulder function score was higher than that of the control group after rehabilitation training in the observation group,and the fracture healing time was shorter than that of the control group,and the differences between the groups were statistically significant(P<0.05).No adverse effects occurred in both groups.Conclusion The therapeutic effect of early rehabilitation combined with intramedullary nailing is ideal in treating proximal humerus fractures and can effectively promote fracture healing and shoulder joint function recovery,and it has high safety and is worthy of clinical application.
