高精度经颅直流电刺激对精神分裂症患者认知功能及社会功能的影响

Effects of High-Definition Transcranial Direct Current Stimulation on Cognitive Function and Social Function in Patients with Schizophrenia

摘要 目的探讨精神分裂症患者采用高精度经颅直流电刺激(high-definition transcranial direct current stimulation,HD-tDCS)对其认知功能及社会功能的影响.方法选择2018年12月—2020年11月该院收治的精神分裂症患者88例,采用随机数字表法分为两组,每组44例.对照组采用常规药物治疗,观察组在对照组基础上采用HD-tDCS治疗,均持续治疗10 d.比较两组治疗前后的认知功能、社会功能以及不良反应.结果治疗后,观察组中文版精神分裂症认知功能成套测验(Chinese version of MATRICS consensus cognitive battery,MCCB)各维度评分中社会认知、推理与问题解决能力、工作记忆、注意/警觉、处理速度评分及总分均高于对照组,组间差异有统计学意义(P<0.05);两组治疗后MCCB各维度评分中视觉学习和记忆、言语学习和记忆评分比较,组间差异无统计学意义(P>0.05).治疗后,观察组住院精神病人社会功能评定量表(scale of social function in psychosis inpatients,SSPI)评分中社会性活动技能、动性和交往情况、日常生活能力评分及总分均高于对照组,组间差异有统计学意义(P<0.05).两组治疗期间均未见不良反应.结论HD-tDCS能够改善精神分裂症患者的社会功能及认知功能,安全可靠,值得临床推广. Objective To investigate the effects of high-definition transcranial direct current stimulation(HD-tDCS)on cognitive function and social function in patients with schizophrenia.Methods A total of 88 patients with schizophrenia who were admitted to this hospital from December 2018 to November 2020 were selected and divided into two groups by random number table method,with 44 cases in each group.The control group was treated with conventional drugs,and the observation group was treated with HD-tDCS on the basis of the control group,and the treatment was continued for 10 d.The cognitive function and social function before and after treatment and adverse reactions were compared between the two groups.Results After treatment,the scores of social cognition,reasoning and problem solving skills,working memory,attention/alertness,processing speed and total scores in each dimension of Chinese version of MATRICS consensus cognitive battery(MCCB)in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05);there was no significant difference between the two groups in the scores of visual learning and memory,verbal learning and memory in each dimension of MCCB after treatment(P>0.05).After treatment,The scores of social mobility skills,mobility and communication,daily living ability scores and total scoresscale of social function in psychosis inpatients(SSPI)of the observation were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).There were no adverse reactions in the two groups during treatment.Conclusion HD-tDCS can improve the social function and cognitive function of patients with schizophrenia,is safe and reliable,and is worthy of clinical promotion.
