摘要
目的研究神经肌肉电刺激与早期被动活动对机械通气患者ICU获得性虚弱的影响.方法选取该院2018年11月—2019年11月收治的机械通气患者80例为研究对象,采用随机数字表法将其分为对照组、活动组、NMES组及联合组,每组20例.对照组采用常规干预,在此基础上,活动组进行早期被动活动,NMES组接受神经肌肉电刺激,联合组则采用神经肌肉电刺激及早期被动活动,观察四组患者的治疗效果.结果干预1、3、7d后,活动组、NMES组及联合组的肌力等级均高于对照组,且NMES组等级高于活动组,联合组等级高于活动组及NMES组,组间差异有统计学意义(P<0.05).干预后,活动组、NMES组、联合组的机械通气时间、ICU室停留时间、住院时间均短于对照组,ICU获得性虚弱的患病率低于对照组,且NMES组各项时间短于活动组,ICU获得性虚弱的患病率低于活动组,联合组各项时间短于活动组及NMES组,ICU获得性虚弱的患病率低于活动组及NMES组,组间差异有统计学意义(P<0.05).结论神经肌肉电刺激与早期被动活动均能够有效改善患者病情,缩短康复时间,帮助其尽早脱离危险期.两种方式比较,神经肌肉电刺激治疗能够更好地降低患者出现ICU获得性虚弱的概率,具有更高的应用价值.而将两种方式联合使用,可进一步提高患者的干预效果,值得推广使用.
Objective To investigate the effects of neuromuscular electrical stimulation and early passive activity on ICU acquired weakness in patients with mechanical ventilation.Methods A total of 80 patients with mechanical ventilation admitted to the hospital from November 2018 to November 2019 were selected as the study subjects,and they were divided into a control group,an active group,a NMES group and a combined group by random number table method,with 20 patients in each group.The control group received conventional intervention,and on this basis,the activity group received early passive activity,the NMES group received neuromuscular electrical stimulation,and the combined group received neuromuscular electrical stimulation and early passive activity.The therapeutic effect of the four groups were observed and compared.Results After 1,3 and 7 days of intervention,the muscle strength grades of the activity group,the NMES group and the combined group were higher than those of the control group,and the grade of the NMES group were higher than that of the activity group,and the grade of the combined group were higher than those of the activity group and the NMES group,the differences between the groups were statistically significant(P<0.05).After intervention,the use time of mechanical ventilation,ICU stay time and hospital stay time were shorter in the activity group,NMES group and combined group than in the control group,and the prevalence of ICU acquired weakness was lower in the activity group,NMES group and combined group than in the control group;the times in the NMES group were shorter than those in the activity group,the prevalence of ICU acquired weakness was lower than that in the activity group;and the times in the combined group were shorter than those in the activity group and the NMES group,the prevalence of ICU acquired weakness was lower than those in the activity group and the NMES group,the differences between the groups were statistically significant(P<0.05).Conclusion Neuromuscular electrical stimulation and early passive activity can effectively improve the patient's condition,shorten the recovery time,help them out of the danger stage as soon as possible.Compared with the two methods,neuromuscular electrical stimulation can better reduce the probability of ICU acquired weakness and has higher application value.The combination of the two methods can further improve the intervention effect of patients,which is worth promoting.
