神经肌肉电刺激联合早期吞咽管理对颅脑损伤吞咽障碍患者吞咽功能及神经功能的影响 被引量:1

摘要 目的探究神经肌肉电刺激联合早期吞咽管理对颅脑损伤吞咽障碍患者吞咽功能及神经功能的影响.方法选取该院2019年12月—2021年4月收治的142例颅脑损伤吞咽障碍患者,按照随机数字表法分为对照组和观察组,每组71例.对照组采用早期吞咽管理,观察组采用神经肌肉电刺激联合早期吞咽管理.比较两组干预前后吞咽功能及神经功能.结果干预前,两组洼田饮水试验(water swallow test,WST)评分、Gugging吞咽功能评估量表(gugging swallowing screen,GUSS)评分、标准吞咽功能评估量表(standardized swallowing assessment,SSA)评分及美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分比较,组间差异无统计学意义(P>0.05);干预后,观察组WST、SSA、NIHSS评分均低于对照组,GUSS评分高于对照组,组间差异有统计学意义(P<0.05).结论神经肌肉电刺激联合早期吞咽管理可有效增强颅脑损伤吞咽障碍患者的吞咽功能,减轻神经损伤,促进患者恢复,值得推广. Objective To explore the effects of neuromuscular electrical stimulation combined with early swallowing management on swallowing function and nerve function in patients with swallowing disorder after craniocerebral injury.Methods 142 patients with swallowing disorder after craniocerebral injury treated in the hospital from December 2019 to April 2021 were randomly divided into a control group and an observation group,with 71 cases in each group.The control group adopted early swallowing management,and the observation group adopted neuromuscular electrical stimulation combined with early swallowing management.The swallowing function and nerve function before and after intervention were compared between the two groups.Results Before the intervention,the water swallow test(WST)score,the gugging swallowing screen(GUSS)score,the standardized swallowing assessment(SSA)score and the National Institute of Health stroke scale(NIHSS)score were compared between the two groups,and there were no statistically significant differences between the two groups(P>0.05);after the intervention,the WST,SSA,and NIHSS scores of the observation group were lower than those of the control group,and the GUSS scores were higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Neuromuscular electrical stimulation combined with early swallowing management can effectively enhance the swallowing function of patients with swallowing disorder after craniocerebral injury,reduce nerve damage,and promote the recovery of patients.It is worthy of promotion.
