经皮椎弓根螺钉内固定对单纯性腰椎骨折患者椎体功能恢复情况的影响

Influence of Percutaneous Pedicle Screw Internal Fixation on Recovery of Vertebral Function in Patients with Simple Lumbar Fracture

摘要 目的探讨经皮椎弓根螺钉内固定对单纯性腰椎骨折患者椎体功能恢复情况的影响.方法选取我院2015年6月—2020年6月收治的单纯性腰椎骨折患者50例,按随机数字表法分为观察组和对照组,每组25例.对照组行开放椎弓根螺钉内固定术,观察组行经皮椎弓根螺钉内固定术,两组术后均随访11个月.比较两组患者的手术指标、术前与术后11个月的椎体前缘高度比、后凸Cobb角、腰椎功能障碍指数(ODI)评分及疼痛视觉模拟评分(VAS).结果观察组患者的手术切口、住院时间均短于对照组,手术出血量、术后24 h切口引流量均少于对照组,组间差异有统计学意义(P<0.05).术后11个月,观察组患者的椎体前缘高度比为(94.55±5.23)%,高于对照组的(82.94±4.25)%,后凸Cobb角为(2.97±1.03)°,小于对照组的(9.06±2.87)°,组间差异有统计学意义(P<0.05).术后11个月,观察组患者的ODI评分、VAS评分均低于对照组,组间差异有统计学意义(P<0.05).结论采用经皮椎弓根螺钉内固定术治疗单纯性腰椎骨折患者可有效降低手术创伤,减轻疼痛及功能障碍程度,改善椎体功能,促进术后恢复. Objective To investigate the effect of percutaneous pedicle screw internal fixation on the recovery of vertebral function in patients with simple lumbar fracture.Methods A total of 50 patients with simple lumbar fracture admitted to our hospital from June 2015 to June 2020 were selected and were divided into an observation group and a control group according to random number table method,with 25 patients in each group.The control group received open pedicle screw fixation,and the observation group received percutaneous pedicle screw fixation.Both groups were followed up for 11 months.The surgical indicators,anterior vertebral height,kyphosis Cobb Angle,lumbar dysfunction index(ODI)score and pain visual analogue scale(VAS)score were compared between the two groups before and 11 months after surgery.Results The operative incision and hospital stay in the observation group were shorter than those in the control group,and the operative blood loss and incision drainage flow 24 h after operation were less than those in the control group,the differences between the groups were statistically significant(P<0.05).11 months after surgery,the anterior height ratio of the vertebral margin in the observation group was(94.55±5.23)%,higher than(82.94±4.25)%in the control group,and the Cobb angle of kyphosis was(2.97±1.03)°,lower than(9.06±2.87)°in the control group,the differences between the groups were statistically significant(P<0.05).11 months after surgery,the ODI score and VAS score in the observation group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Percutaneous pedicle screw internal fixation can effectively reduce surgical trauma,relieve pain and dysfunction,improve vertebral function and promote postoperative recovery in patients with simple lumbar fracture.
