摘要
目的探讨异烟肼联合阿米卡星支气管镜下注药在耐多药肺结核(MDR-TB)患者中的应用价值.方法选择2019年10月—2021年3月该院收治的82例MDR-TB患者为研究对象,按随机数字表法分为两组,每组41例.对照组采用抗结核治疗,研究组在对照组基础上加用异烟肼联合阿米卡星支气管镜下注药,两组均持续治疗6个月.对比分析两组痰菌转阴情况、临床疗效、免疫功能、不良反应.结果研究组治疗后痰菌转阴率73.17%及治疗总有效率95.12%均高于对照组的51.22%、78.05%,CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,CD8^(+)低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论异烟肼联合阿米卡星支气管镜下注药能够有效促进MDR-TB患者痰菌阴转,调节其免疫功能,且无严重不良反应,值得临床推广应用.
Objective To explore the application value of isoniazid and amikacin under bronchoscope injection in patients with multi drug resistant pulmonary tuberculosis(MDR-TB).Methods A total of 82 MDR-TB patients admitted to our hospital from October 2019 to March 2021 were selected as the research objects,and divided into two groups according to the random number table method,with 41 cases in each group.The control group was given anti-tuberculosis treatment,and the study group was given isoniazid combined with amikacin under bronchoscope injection on the basis of the control group.Both groups were treated continuously for 6 months.The sputum bacteria negative conversion,clinical efficacy,immune function and adverse reactions were compared and analyzed between the two groups.Results After treatment,the rate of sputum bacteria negative conversion in the study group was 73.17%and the total effective rate was 95.12%,which were higher than 51.22%and 78.05%in the control group,CD4^(+),CD4^(+)/CD8^(+)were higher than the control group,CD8^(+)was lower than the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Isoniazid combined with amikacin under bronchoscope injection can effectively promote the negative conversion of sputum bacteria in MDR-TB patients,regulate their immune function,and have no serious adverse reactions,which is worthy of clinical application.
